I want to know what would be the ultimate grief to be prepare to avoid and fix it in case it happens.

Either something with worldedit or tons of TNT.

Watch griefing videos to get an idea.

How would someone without permission to use it do that?

If its creative server, they can get it easily. If it is not, they would have to gain trust.

simple, replace all blocks on all layers under the top layer grass / sand with TNT and then put random pressure plates in places people would usually walk on the most, then watch the whole place explode and the server crash when someone walks on one of them....

I'd never grief anyone other than a friend I played with on a private server. In order to grief him, I created challenges with signs, making a puzzle. Failure meant explosions or death, success meant... well, survival. Sort of like logging in to a SAW scenario.

Sadly... Though I tuned them to his own unique personality to present a challenge, circumstantially, he always blundered through, neglected to read the signs, and blew up...

Serves him right for putting noob towers up in my yard or making my lava and water features within my foundry combine... >.>

Two options.
Option 1:
Remove every block for 5000 blocks in each direction from the spawn, on all Y levels, except for a single block at the spawn itself. This includes bedrock. (Imagine it... guy logs on, another guy logs on, they get into a SHOVING WAR)

Option 2:
Every block in the spawn chunk replaced by TNT. Ignite. Suddenly, logging into the server's spawn causes a server crash. (Verified by me. Did it once on a "creatisurvival do whatever the heck you want, grief whatever" server where I was an admin. [Still am. :P])

Schroedinger's TNT.
Until you look at the huge chunk, you have no idea whether it is exploding or not.
(Obviously a pressure plate on the inside would cheat the experiment and turn it on)

Creeper spawner in the roof, where NO-ONE expects to find it. And a hack to remove torchs, lightstone, redstonetorches, jak o' lanterns and stuff. MWAHHAHAHAHAHAHAHA!
