As far as minecarts themselves I'm pretty sure it's a simple problem, since serverside they actually work perfectly-- it's just that when the client right-clicks a track, it SHOULD only place the cart serverside, but instead it places it clientside as well, and the two interfere and spazz-- this is where the "duplicate" comes from. If you'll notice, only the person that placed the minecart sees the duplicate.
I think the problem with the player spazzing inside of them is merely due to notch's anti-wall hack code, which sees the cart's movement as a violation of the norm. If I can disable that for players inside a minecart, and find a way to stop clientside minecarts from being placed, there'd only be one problem left-- the infinite death spazz which seems to be due to minecarts having infinite health. Not sure how to fix that last one-- I can't steal the code from the boat, since the only reason boats work is because they're only clientside.
The minecart fix is actually amazingly easy. It's in the ia.class with some of the other stuff.
Simply increasing the distance moved check seems to fix the spam.

Anyone know how to fix minecarts? Running Hey0 mod + Llamacraft 3.6. TY.
I'd like to know as well... even if the fix is client-side and I just run a private server in offline mode with updated clients only
I've looked at the ia.class but the code is rather confusing with the shorthand that Notch used I guess
