Capes are supposed to be a special gift from Mojang. They don't add anything to the game or make a player more powerful, they are purely visual. If they were given to everyone, they'd become completely pointless and boring. I don't like capes because they look nice, but because they're hard to get so if you do get one, you treasure it.
Something like this would be rather easy to add to normal survival using mods or a command block module, hardcore mode is suppose to be, as the name suggests, hardcore. If you would like, I could probably whip up a one-command to add to normal survival that makes it so a ritual is needed to revive for you. But as for the suggestion, no support.
You don't use colons in vanilla commands. If you use a space instead ("/fill <x1> <y1> <z1> <x2> <y2> <z2> minecraft:stained_glass 15"), it should work.
The way to do this would be by using scoreboard operations. At the beginning of the repeating command block chain, you would have the command,
/scoreboard players operation PLAYER DUMMY_SCORE -= PLAYER HEALTH_SCORE
and at the end of the command block chain, you would have the command,
/scoreboard operation PLAYER DUMMY_SCORE = PLAYER HEALTH_SCORE
In between these commands you would have the commands you'd want to run by executing at that player with "score_<dummyScoreboard>_min=1" in the selector. For example, if you want them to say "Ouch!", you would do,
It seems like this would be better for a plugin then added to vanilla minecraft. As you said, this would help factions and anarchy servers, but those can use plugins fine without affecting anything. I pretty much only play whitelisted strictly vanilla survival servers, as I don't enjoy pvp or getting my builds griefed. I already destroy my own builds often, because I either want to move it, change it, or upgrade it, and with the amount of minecraft I play, misplacing blocks is unavoidable. This would greatly reduce the stuff my friends and I can do, as it increases the time we'd spend breaking blocks.
Often times we also break blocks others place as well, not to grief, but to help them build or to take down a prank (the actually fun kind, not the practical joke kind) that someone else pranked us with.
As much as I'd like to help you improve your idea, there's nothing there that would help my type of play, only hinder it. Increased safety is always at the cost of freedom, and I play minecraft for the freedom of being able to do anything.
Witch spawns are very rare, even in witch huts. I've never played MCPE, so I don't really know whether or not witches spawn, but in MCPC you have to wait around one for several hours before even one spawns.
There's also been rumors of pistons being able to push block entities, because of the GIF posted about MCPE. Even though it's not confirmed for PC, I think there should be a section in the OP for possible changes.
Their job as game developers is to entertain us, their community. Sure, the April Fools jokes don't last for years, but they are most definitely entertaining, and I for one am excited to see what they're going to pull out of their hat.

Of course, if you actually wanted them to say ouch, you'd probably want to use the tellraw command, the say command was just an example.
