Minecraft loads without a hitch, but it crashes when I open a world, a minute after I open a world, or when I create a world. No ID conflicts, though some interesting things in the crash report...

It seems like it didn't paste all of the log. I want you to clarify one thing. Does it say at the end of the crash log something to do with "leaking world"?

No, it said ticking screen/exception in world tick. I believe two mods are clashing that both affect villagers, as it crashed every time a villager came up (Minecraft Comes Alive) or when I spawned one. I'm going to go check if any other mods affect the villagers or not, as I could play perfectly fine without the villagers around.
Look through the log. If it was IDs, it would have been "Block __ is occupied by block ___"
Also, I'm pretty sure there aren't 93 mods that use Modloader, and if there are, they are lame "weapon and tools" adding mods.
The only Modloader mod I have here is the Herobrine mod.
