ssmci.exe is the filename, defaultly installs to Windows\System32 but you can technically place it anywhere (Though it won't show up in the screensaver list).
Since it works on Vista, there should be no problems with 7 as 7 is just a modification of Vista rather than a complete rebuild. Hopefully(for me anyway, not you) you have an Intel card and 1.3's fix will make it work.
This is rather strange... I did a search on my System32 folder for ssmci.exe, and got nothing. Same for the Windows folder. But I can see it in the screen saver selection window... And I have an nVidia card, laptop. If you need specs or any information let me know.

I can run ssmci.scr and it works fine.
I assume the UAC may be blocking it. I am not sure but if you right click on the Screensaver's file and goto properties is there anything in there to tell the UAC to run it in Administrator mode?
P.S If you up for it you could temporarily disable the UAC to see if it truly is the problem.

4. when using the screensaver select window MCI appears as the desktop background with win7 screensaver icon over the top. cannot access settings or preview.
Could you extract This to Windows\System32\ and see if it works. Iv modified the Manifest file so it doesn't request Admin rights. No clue if it will help or not.

I have windows 7 and I cant get mine to show anything. Ive installed it but I cant figure out what to do now... Nothing happens when I choose it and press preview or settings.
Good. Seems like the screensaver selector can't run applications that request administration rights(Which i generally make my apps ask for it as without it you can then have errors if your app tries to do something that the UAC thinks should require admin rights). Oh how i despise the UAC.
The installer has been updated to now include the updated ssmci.scr . No other changes.
@SkiPiiT and Galaksee: Please re-download the installer(Or overwrite the ssmci.scr with This (in Windows\System32\)).
Nice screen saver. I haven't messed with it too much, but I like it.

I'll just provide suggestions big or slow, so meh. You could implement a feature where it could show a part of your single player world, or multiplayer one if it's even possible. You know, like some kind of tour or whatever. Feel free to take it into consideration or ignore it if it's too difficult. You could always make a challenge out of this.
I'll have to say though, I don't know what's needed for a screensaver, so I'm just saying stuff.

A Screensaver is just a normal application that has a scr extension rather than a exe one. Basically if you renamed the Minecraft Launcher application to have a src extention and placed it in Windows\System32\ you could have it as a screensaver (Though it wouldn't be fullscreen and it would launch each time you selected it from the screensaver list).
Im using this as the background for the spinning block.

Most Options can update the Preview Window in real time.
Background Tile now displays properly in the Preview window.
Hints added to certain options (Ones that may not be obvious)
Use Menu Background added to the 2 Modes (Forces the background to look like Minecraft's Menu)
Block Map's background is now a checker pattern instead of Fuchsia
Random Fall Chance and Random Fall Spread options added to allow users to control how the blocks fall.
Subtitles added.
Default options of some sections have changed.
Its recommended to press the Default button on the Minecraft Logo tab as there are differences between the default values in previous versions. Note that if you set the Random Fall Spread to 1 you will get a nice side sweep pattern.

Thank you for the great screensaver, is there any chance you could make a second download that installs the two screen-savers separately instead of alternating between them. I find that the one that replicates the opening screen with the falling stone blocks is VERY laggy... And I generally just prefer the rotating block anyways. Normally I just go without a screensaver but if this was an option I would definitely use the rotating block screensaver.
