It's a straight line function, so it'll be of the form y=mx+b. As said above, you can find the slope as m=(y2-y1)/(x2-x1), then you can substitute x1, y1 into the function and solve for b.
For a more detailed explanation, see here (it uses a slightly different method, but the result is the same. According to wolframalpha, all the points lie on the curve, so you can choose any two points for this.

The x value increases by 20 and the Y value decreases by 5, so I'm guessing that x = -4y plus or minus some random number.

1.5 = -0.25*(-12) + b
1.5 = 3 + b
b = -1.5
Therefore f(x) = -0.25x - 1.5
I can confirm that this is correct. I just mashed each x-coordinate into the equation and got the correct y-coordinate with my calculator.

That's the forumla to find the slope itself. Slope Intercept Form looks like y=mx+b. If your teach wants point slope form, it's (y - y1)=m(x - x1)

Well I just checked if it works for every point and indeed it does so it should be right.
I also correctly identified that random number as -1.5
I win.
You also went as far as to say it was -4/x which is incorrect; it's not the same thing as -x/4
