Double click on the Optifine file you downloaded. It should install itself. If it doesn't ensure to play minecraft alone in the version you intend to play, THEN try again.
Play the game with Optifine once.
Open up the base minecraft folder, and look for a "shaderpacks" folder that should have been created. Just put the shader file there.
Open up the game, enter your world and select "Video Settings" > "Shaderpacks" and select the shader from the list. Enjoy!
NOTES
You can turn off the yellowish tint when on full light by clicking "Shader options" in Optifine's shader selecting screen. Same goes for blue-ish darkness.
There are also options to disable toony-like light spreading, light coloring and one to make outlines softer/lighter.
You can change even more configurations at the "final.fsh" and "gbuffers_textured.fsh" text files inside the shader.
Don't forget to report any weird problems with the shader. There might be some issues with transparent textures.

This shader is held under the Attribution 4.0 International license. You may use it and edit/mod it freely, so long as you copy-paste the "LICENSE.txt" file found within the .zip.

I have tested this shader on another computer, and it seems to yield different results than those I get on mine.
I'd like people to help me correct it by telling me how does it look on your computer.
Does it look like in the screenshots? Does the outlining look the same as in the pictures? Does it have any issues with sky rendering (weird clouds, weird dark line in the horizon...)...?
Any volunteer testers and problem reports would be much appreciated. Just download from direct link for now.
Please let me know what minecraft (and optifine) version are you using, and if you really wanna help, issue a screenshot of how it looks in your pc, and specially, screenshots of possible weird glitches you get, if any.
You don't only have the hand issue, the whole shader looks incorrect in your screenshots. In my computer it looks completely different (like in the screenshots from the post), but in my laptop it looks different both to that and to what you're showing me.
I'm going to try and move this to the WIP section tomorrow night as it's clearly lacking a lot of work on compatibility.
I will have to ask for help to somebody who knows GLSL better and how the different .fsh and .vsh minecraft files are passed on within the pipeline.
For example, take a look at this screenshot from a version of the shader I'm working on (see attachment), as tested in my laptop:
Both the depth map and the normal map of spiders (and the blocks it occupies?!) gets random noise, causing the random "dust" you see in the picture as the edge detection algorithm goes crazy. In my "good" PC it doesn't do that.

I am having the same problem
Because it is outlined even is see-through the hand... I am gonna figure out what happened... This bug was discovered by: SamhuffyX4.
