I've drained the monument, gutted the inside so there's absolutely no walls left, still can't find the last elder guardian. I've looked all outside in case it slipped out the front door, but can't see anything other than swarms of normal guardians. Now I'm looking underneath the monument as I can't think where else it could be.

Yes still getting mining fatigue which is how I know there's still one. It only affects me near the bottom of the monument. I've only left the floors and the outer walls, even put windows all round each side to double check that I cleared everything. I have moved a cow into the monument to save time, but it's so frustrating not being able to find it! There's literally nowhere it could be except outside or underneath. I don't think...

I'll go back to exploring underneath. Just if they definitely can't leave the monument then it must be some kind of glitch and I'm wasting my time. But if they can, I might have to end up drying up the ocean around the monument too till I find it. Hoping someone can confirm either way. Oh and I came through the roof, so I know I've explored every level.
Well the range of mining fatigue is supposed to be 50 blocks so that would seem to indicate that the elder guardian is in a cave way down under the monument. I'd suggest marking the locations where you receive the effect and digging tunnels down and out 50 blocks around.
Do you have a conduit?
I seem to remember mining not being too bad with a conduit counteracting the mining fatigue as long as the tunnels are dry.
Though that was in an earlier version.

I guess I'm confused, the conduit only works if you are in water.
But I think you could still get the best of both worlds if your feet are in water and the upper part of the tunnels are dry.
If all else fails you could use spectator mode or switch to peaceful to despawn it if you don't have other hostile mobs you want to keep like elder guardians in nearby monuments or name tagged hostile mobs.

I've spent so long on this now it's feeling personal, so I'm kind of committed to the hunt. The 50 blocks thing is a good idea, I'll try to map out where it hits/doesn't hit me. Thanks.

Well I dug down under the monument, nothing, so I ended up using spectator mode. And amazingly I finally found the elder guardian, underground, and some distance outside the perimeter of the monument! It's quite hard to see it in this screenshot but it's about in the middle of the screen, lower than halfway down. You can see the ladder I was using to dig down in the background, and a few of the green monument blocks. Now I just need to mark it's location so I can go and find it in normal mode.
I don't know if it's just me and my monitor but I can see the first picture of it very well if it's at the bottom of my screen or if I stand up, so that I'm looking down at it, looking straight it's very dark and looking upwards it's almost totally black.
(I have the opposite problem with google maps, I can only see the faintly shaded silhouettes of the buildings if they are at the top of my screen.)

Oh, and if you still can't make out the screenshots, there is a big cave system starting out from just under/to the side of the monument, including an underwater ravine. The monument itself is intact, so the guardian must have gone out the front door and then there are holes and waterfalls and things leading to where it's now stuck.
And it's stuck at level 20, whereas the bottom of the monument is at level 40.
I'd blocked up the doorway quite early on, so maybe it had slipped out by accident then couldn't get back in.
Shouldn't be hard at all to get it in a minecart where it's at. I'd nametag it and cart it off somewhere remote (maybe the Nether) for a really cool and unique prize.

Sounds long and complicated to me, so I'll just make my way to it, kill it before it kills me and then enjoy my new home - has to be my new home with the amount of effort I've spent on it.
And one last thought about how it got there - I used sand to clear out the monument, so it's possible I forced it toward the front door without knowing, and then out the door. Though I've used that method to clear them before and never had that happen, but possibly I've taken different routes before. Might be fun to try to do it on purpose another time and see what happens.

And in case anyone is interested, I finally reached it and killed it. I mined down one of the ocean monument support legs until I was at the right level, then made myself a glass tunnel, much to the bemusement of the random guardians down there, and then made a door to stick my head out of and shoot. Took 40 arrows with a basic bow.
I will now put a sign up to mark the location. Could be a tourist attraction.

There should be one in each wing and one in top, just under the very upper roof.
I really don't think they move around the monument, I think they stay in their respective rooms.
I can't think of any way they could get out unless you remove the walls/roof.

The water sort of obscures it.

Sorry about that - I have my monitor set to it's brightest which is probably why I can see it. Though I can only just see it myself anyway.
I've just decided to make some pumpkin lanterns to light my way. I have rubbish armour and no underwater gear, so I need all the help I can get.
