Or, if you mean just get multiple enchantments on one tool, it's random. The higher the level, the bigger chance you have of getting multiple enchantments on that tool.

The only way I know to enchant the same item twice, without using mods to remove the enchantments, is to use the repair feature, combine it with another of the same item. Preferably one with very few uses left, then enchant it again.
You can cycle through the level options in much fewer clicks.
Allows you to add/upgrade enchantments to already enchanted items.
So you can always work towards the enchantments you want without being sabotaged by random chance.
Allows you to undo crappy enchantments Most of your xp is refunded, (with the probability of that enchantment reduced, until you close the table again).
Much fairer enchantment costs (based on level caps and min-enchantability)
As of 1.4.2 with the anvil you can combine 2 tools to add more enchants. so technically you can't double enchant one tool but u can combine 2 of the same tool and combine their enchants so technically technically yes.
You can't enchant an item more than once but you can increase the chance of getting multiple enchantments by Leveling up and by placing bookshelves around your enchanting-table. The other way of getting multiple enchantments is to combine enchantments by an Anvil, for example I've an Iron Pickaxe Efficiency III and another one with Unbreaking II, if I combine them into one pickaxe by using an Anvil then I'll get a pickaxe with Efficiency III + Unbreaking II. But both have to be the same tool made of the same material. You can also enchant a book and then add its enchantments in the same way as described above (I got Power III and Punch II in my bow with the same method), but this method costs much more levels than enchanting table and it depends upon the level of enchantment.
Pretty sure you just slap it back on the Enchantment Table and re-enchant. BUT ----- YOU LOOSE WHATEVER WAS ON THERE!

Say you enchanted that sword level 30 and you get Looting 2 and Sharpness 3
You want to improve those enchantments, and so you go to your anvil, and there's two ways to enchant.
1. Put two swords/ other tools/armor with similar enchantments eg. you have a second Iron Sword with Sharpness 1 and Looting 2, you can join them together to put their enchantments up by one, so your Sharpness 3 becomes 4, and Looting 2 becomes 3 for a certain amount of enchanting points.
2. Put a tool/armor in an anvil with an enchanted book, and for a certain amount of enchanting points, you'll get that enchantment on the tool/armor

if you repair the item, it loses the enchantment - then you can attempt a new enchantment
you cannot add enchantments to a previously enchanted item
if lucky, you will get more than one type of enchantment

In old Minecraft you cant get enchanting books I guess, I never played 1.4, only 1.5+
