So I found myself a haven of safe diamonds, tons of redstone and a reusable Obsidian set, I am working my way to try and get back to my main house what happens...GRAVEL!!!!
I lost a 45 diamonds, 3 full 64 sets of redstone, and my diamond sword, diamond armor, diamond pickaxe, and all the EXP I had almost 30.
I was trying to climb using 3x1 so opening 3 above me with one wide and I was Just over the ledge I guess when I broke the cobblestone and 14 blocks of gravel dropped on my head and by the time I got back to the spot it was all gone.
I just came back from a caving trip with 31 diamond blocks (and a couple thousand coal blocks, plus a double chest of iron ore); surely I am not going to stop what I'm doing and run back home every time I find some diamonds (in fact, I use a backpack mod to minimize this, hence the double chest of iron ore, actually even more since I had 4-5 more stacks in another backpack since I didn't want to stop until I ran out of wood for torches; I came back with a total of around 70 stacks of iron and gold ore).

That sucks. It happened to me once but I wasn't carrying as many and I got them back. As others said, place a torch below you (or just don't dig strait up).

It is otherwise impossible to explore the caves in my world; here is a comparison between vanilla and my world (before I explored anything), both the same scale, and a map of caves I've explored so far (same area as in the top image, but rotated to the left 90 degrees to show caves better; the giant cave system in the top image is on the left side with the stuff to the right at the bottom of the top image), to give you an idea of how vast these cave systems are (note too that the ground is 3 times deeper, such that cave systems are as tall as they are wide):
So far I've mined around 40,000 ore from those caves, using 17,000 torches to light them up (you can actually see them in the second image). Stuff I lost from dying? Zero, with just one death while caving because I fell down a 100 block deep ravine (of course, Rei's Minimap has death points but that won't save you from lava but it is easy to avoid lava if you liberally use a water bucket).
