I believe there are many blocks/items that are rare. I would defiantly say things such as the ender dragon egg, (or eggs, if you're a glitch guy) or blocks such as Mycelium, Podzol, and other 'special blocks'. I will not say emeralds because you can trade with villagers to get emeralds. There are probably more items/blocks, but that's all I can think of right now.

Naturally spawned pink sheep (I have only seen about 2-3 naturally spawned pink sheep and usually its just after spawning)
Emeralds (if you spawn in a world with a seed that doesn't include a village, roofed forest biome, and/or extreme hills biome for miles)
Monster Spawners (I have seen less dungeons generate in current updates than in older updates, but I guess my luck in finding dungeons might not be very high anyway)
Emeralds are only rare depending on where you mine. I did a lot of mining in extreme hills biome, and have over a stack of blocks of emeralds.
You can only find emerald ore while mining under extreme hills biomes or roofed forest biomes. Emerald ore does not spawn anywhere else, and they are found by themselves and never in a vein
Monster Spawners (I have seen less dungeons generate in current updates than in older updates, but I guess my luck in finding dungeons might not be very high anyway)
Dungeons, as well as mineshafts (cave spider spawners) were indeed made much rarer in 1.7; dungeons were made rarer largely because they increased the y-range they can generate over from 128 to 256, presumably since terrain could now go higher, but this halved the chance of one generating below sea level, where the vast majority of terrain still is, as well as caves themselves, which were not adjusted either (caves do not even generate above y=128, with only the very rare cave that extends above that point). It also doesn't help that mineshafts, which have a structure which is highly favorable for dungeons, were made 40% as common, which is also about the overall frequency of dungeons and mob spawners. Or that cave systems were made much smaller and less interconnected, making it harder to explore a large area underground (over the past few days I've explored three large cave systems and 2 mineshafts, plus many ravines, all part of a vast interconnected network, with many dungeons found. On the flip side, they aren't that common in the largest cave systems because there isn't enough solid ground for them to generate in).
Even then, I only find about one mob spawner per hour spent caving; 453 in 466 hours in a recent world, including 213 dungeon and 240 cave spider spawners, though I also only found 92 emerald ore, compared to 1598 diamond ore and 380131 of all ores in the same time (in my first world the diamond:emerald ratio is much lower, around 5:1, due to a higher proportion of emerald-containing biomes). Of course, emeralds themselves are not rare at all; I bought more than 800 in order to get Mending books for my gear and more than 5,000 to get diamond gear for repairs in my first world.
None of this compares to the 27 chiseled stone bricks that I've taken from 9 jungle temples in my first world (not craftable in 1.6.4), from an area of about 90,000 chunks (for comparison, such an area has about 2,250 dungeons and 900 mineshafts in 1.6.4 and 900 / 360 in 1.7, not including a reduction in mineshafts within 1280 blocks of the origin).
None of this compares to the 27 chiseled stone bricks that I've taken from 9 jungle temples in my first world (not craftable in 1.6.4), from an area of about 90,000 chunks (for comparison, such an area has about 2,250 dungeons and 900 mineshafts in 1.6.4 and 900 / 360 in 1.7, not including a reduction in mineshafts within 1280 blocks of the origin).

everyone says diamonds, or emeralds, discs, ETC. WELL NO, in reality, its mycelium, only found one layer of an extremely rare biome, while diamonds spawn everywhere, but mycelium is a block, not item
or if you want to get realistic, the rarest item is...notch apples, only obtainable in dungeons, every enchant in the game maxed out on it
I would say the rarest item is probably be the dragon egg, and then the second rarest is cocoa beans, with the third being something like endstone or ghast tears.
The only reason i didn't put mossy cobblestone in it was because i dont even see the point of it.
I'd say the rarest item is the Elytra because it's the hardest but yet the most useful item that can be obtained in the End. But endstone probably isn't the rarest because it's found all over the End.
