This will be the start of the guide on how to survive in a Minecraft PVP server the legit way...
Step 1.) The spawn...
When you first spawn you will most likely spawn in protected land the key is break away from all people and head off in a direction (any works)
Step 2.) The distance...
Everyone knows the first 2000 blocks or so is not safe at all, yet this is PVP so you do wan't to be able to raid other people, I would say a safe distance of around 5000 if you wan't to be extra safe go 10000 away.
Step 3.) The Home...
Building above ground is the worst idea ever and staying in the same base for a long time is also a bad idea, if I were you I would go super far away to build a chest room.
Step 4.) Mining...
Before you get into any battles you must be ready with armour, iron would be best. Also, get those diamonds!
Step 5.) Enchanting...
You will need to get some enchanting done if you ever wan't to prevail. The best option is to build a small sized mob trap (18x18) that only took me about 15 minutes! The drop to kill a mob is 24 blocks you will want it one less than that (slabs work best). After, you can start farming that exp and arrows.
Step 6.) Potions...
I find it vital to have a few Instant Health potions on you and Instant Damage splash potions. If you are feeling like you need it go ahead and make some slowness splash potions.
Step 7.) The encounter...
If you are lucky and you are the one not seen you may be able to pull off the sneak attack and maybe place some lava under him! If not you both may meet head on. When you start your battle make sure you have everything you need on you such as weapons, armour, potions, lava bucket. I normally start off with some arrows if we are not to close, but if you are close go ahead and hit him with some potions and start hitting him with your sword. Jumping while in battle helps because it provides you with more damage towards the opponent.
Step 8.) Looting...
If you found their base you best be moving fast, they may back. After you finish looting rid of their base essentials and leave.
Step 9.) Beast mode...
Continue this gain allies and win.

Make some other potions such as Regeneration, Swiftness, Strength, Fire Resistance, and use these before you enter the PVP area! If you use both redstone and Glowstone Dust on these potions they are alot more effective and last alot longer.
My general inventory for PVP'ing consists of:
1. Diamond Sword, Sharpness IV
2. Instant Health II
3. Instant Health II
4. Potion of Poison II
5. Potion of Slowness II
6. Potion of Harming II
7. Potion of Harming II
8. Golden Apple x3
9. Cooked Beef/Pork x10.
I always try to sprint past whoever i'm going to be fighting and luring them out further into the wilderness, this is a brave move and should only be done if your assuming that you're going to WIN!
But , i always do this, i don't like the fact that the majority of players in the PVP world sit on the edge of the safe zone, waiting to run back in whenever they take damage lol (this may not be possible on other servers).
When i lead them into the wilderness, i tend to find a small 2 block part of the land that i can hide behind to equip my potion of slowness , as they're approaching - hit them with the slowness followed by poison and 2 harming potions, with any luck this should have their health bar half way down already.
then it leaves you for a quick finish with your sword.
As my diamond armour is enchanted to Fire Protection IV and Protection IV, i win most of my fights. & Having a Sharpness IV sword does help when dealing damage ~Big thanks to woNk1L for the HUGE tip
A good defense for an underwater base is to make so that there's a sistem that theres hidden TNT in some room at the top of it, there's a pressure plate to an empty room (with iron door), that pressure plate activates redstone that will activate some TNT in the walls, exploding them and letting the water in, that way, the invader dies, and if you are clever enough, hide your stuff deep under the base.~AntonioTheMiner

Also, for the distance away from spawn you have recommended , that may be acceptable for a 500 player server.... but realistically, most servers are around 25 - 50 players, so i'd say that you shouldn't venture too far away from spawn... however, be wise with your locations and pick somewhere that people tend not to go to... i.e the ocean, if you build your base underwater, the chances are most players won't risk drowning to dig away at the sea bed and find your base.... lol
the best way to start an underwater base is to either:
1) go across the surface in a boat looking for air pockets in the sea bed, usually it will be a natural cave system - more than likely they will have a mob spawner so it's ideal for building a base.
2) dig 3 blocks down and place a block above your head to create your own air pocket.
3) start at the beach and dig a hole quite far down, make sure you cover it up above yourself.... then tunnel out towards the ocean until you think you're far enough out !
I play on a 40 player server, my base isn't too far from spawn and i'm enemy to EVERY other faction on the server, and never once has my base been compromised. However, i do take precautions and bury my items somewhere close to my base.
Ya I agree with a 40 person server underwater base, etc. I have done an underwater base about 1,500 blocks away but still got raided (Mostly because of hacked clients)

You should add something about dealing with teams.
Some people could be by themselfs and get jumped by 2-4 people.
Gotta teach them how to defend themselfs instead of running.
I play on a server called mcpvp.
Its always teams, rarely 1v1s
It's pretty easy to take 2-3 of them down by yourself, depending on their experience in PvP
With just using your sword and your soups.
I used to play on a server and I had like 200 diamond blocks! I also had a team of like 6 people with pro 4 armor and sharp 4 knock 3 and fire 2 swords! Really...it took us like 2 months to get it
The best weapon you can get is a Sharp V Knockback II FireAspect II Looting III diamond sword. They arn't too hard to make, if u have the right requiorments. Best armor set you can get is 2 pieces of Pro IV and 1 Pro III....the other piece is Fire Pro IV or Projectile Pro IV. ( The max pro u can get is 11 not 16)Also having aquaaffinity I respiration III Pro IV is good too. And featherfalling IV Pro IV boots!
10,000 blocks is just the minimal distance. It is not really safe at all. 30,000 will do.
Underground bases are good, but skybases at the building limit are even safer.
Don't build with anything except obsidian, or you will be nuked.
Never use iron armor or weapon, always use diamonds. Also, never go in without any enchants. Use weak enchants if you have to, but those enchants will give you the edge.
For weapons and tools, always enchant at level 30.
"Avoid plugin servers." No, go for plugin servers! Using /spawn comes in handy to get to the spawn, and using /f home of Factions lets you get back to your base for easy raiding. McMMO is an awesome plugin that enchances combat.
