I once was looking for cows, I saw enderman then i tried staying away, while mocing backwards looking at enderman i fell in a cave lol ... sadly i had 20+ diamonds with me and it was so far where i died xD
The last death I can recall was from being killed in SMP the moment I logged on to the server. Ah, if only they had taken more time to write that it was a PVP server, it might not have happened.....

finally found a horse on a mp server , while taking it home during the night i looked back to see if the horse was still attached to my leash all i saw was a charged creeper blow up in my face.

I've been playing the Hexxit mod in MInecraft for the last month and thought I would try the Nether - either it's be no different or hugely different. It was different! Went in god goodies, came back, went a second time collected a lot of goodies and that's when I ran into trouble..
I was fight of one of thos giant heat spiders, now them themselves aren't to bad apart from setting you on fire, I can handle them, They are not what killed me. Being on fire and attacked I couldn't see where I was hitting for the flames... and I accidently hit a zombie pigman! They kiled me repeatedly... Forgot how long they hold a grudge. I went back empty handed,with new swords, but everytimew those damn pigmen got me. I rage quit.
I haven't played it for about a week now, but tried it this morning knowing pretty much everything would be gone in the nether, including my backpack 'o stuff. Went back with a giant Gold sword (All I had left - I had no smelted iron, diamonds were in the backpack) and had to keep fighting off an angry skeleton. Still had to kill a few pigmen but had seen my stuff was still there when I last died. I could have had much time of the 5 minutes left (Especially from last time) - 1 or 2 minutes best. Picked up all I had left and resolved my self that - even though I had lost my backpack and the good stuff I still had my Chocobo back home...
Out of the corner of my eye I saw some xp or string a little bit lower down in a dip and next to it was my backpack! It lives! Mad dash to decide what to throw away but got all my stuff back and lived another day!

I was farming listening to music. It took me 9 and a half hearts to realise a skeleton was shooting me. Now, how I will die. I'll make a TNT trap and set it off.

I offer you advice in the form of criticism. You may follow it, or ignore me in a naive and blissful state.
A creeper fell on my head, blew up, sent my flying upwards, on I died. Whilst on my lava bridge. While carrying my only food source other than mushrooms.
Did I mention I KNEW the creeper was coming but I didn't do anything about it for some reason?

An Enderman knocked me off a ledge in a ravine the other day (luckily no lava since I pour water down the side, just a long drop); falling is my most common cause of death and many cases could have been avoided if I remembered to put on my Feather Falling IV boots (Creepers are second, but only when I'm already injured and one comes up behind me while I'm fighting other mobs, or blowing me off a high ledge).
Of course, the worst time I died (and lost stuff, I use Rei's Minimap for its cave mapping but it also has death points) was when I fell into a nearly hidden 1x1 hole (behind a 1 block high/wide rise in a narrow cave) into a giant lake of lava, died before I could break the surface and dump my water bucket (the iron armor I was using was mostly depleted and broke, I now use better armor with Protection IV). Luckily that was before I started using a backpack mod and used an Ender Chest for extra inventory (RIP to my uber-tools though, at least I had plenty of enchanted books and materials to replace them, just had to spend a lot of time getting XP since all I had for a mob farm was a dungeon).
