what's there to be afraid of? even on my first night in survival, the first and last time i died, i wasn't afraid; i got blown up, so i ran back to where i died looking for a monster that had blown itself up and ended up killing many others throughout the night.
however, less on the fear side, but when i'm running through dark caves and that random sound starts, i've been startled by it when i've been without sound for 10-20 minutes.
Skeletons shooting is my worst fear. Sometime back in beta when iron armour seemed to be such an expensive resource investment and when I built things from wooden planks only.
It was one of my very first nights playing. I had my headphones on and no one was home, I decided to play on normal difficulty to have a go at all these mobs like creepers and such, so I started a brand new world. Naturally, I wasn't too concerned with mining at that moment of time because wood seemed to be the main priority. Come first night, I decided I had best go out and get some coal. So I left my abode (6x6x3) and went to the nearest cave, I saw few zombies in the desert and wasn't too concerned with those, but as I came up to the cave, it just turned out to be one of those natural-awesome-looking tunnels. I was hit from behind by a zombie and immediately ran into this cavern/tunnel. I saw coal and built a quick little shelter around it so I could mine it safely. However, once the resource had been depleted and I made a good 40-50 torches I decided time to leave. I neutralised zombies and spiders of the kind simply because they were common. On a cliff face stood a light grey/white humanoid thing with a bow. I crept closer to discover what this was. I got fairly close when it swung around and shot me making the most crapping-my-underwear-impact-sound-of-the-devil sound ever. I literally skipped a heartbeat and ran back to the cavern/tunnel with the devil's spawn following. I got back into the place where I had mined the coal, closed the entrance off and lit it up. I sat in that little building for a good 10-15 minutes listening to the sound of bones clanking, zombies groaning and spiders hissing.
I decided to take a chance and run back to my wooden house. (This was before the sprinting update sometime early last year so it was natural walking speed).
With my house being in the forest and with no lighting and with nightfall imminent again after I had spent all my second day crying in that coal-makeshift-thing. I ran for my minecraft character's life. Of course, the spiders, skeletons and creepers where still out in the cavern I made it out suffering a good 2-3 shots from the skeleton who, when came into the sunlight burnt to a crisp. I felt I was safe for the time being, in the forest there where a few creepers of which I decided to circle around. When I had found my house finally, night had just set in. I walked inside my dark wooden house and what was waiting for me?
A skeleton with the intentions of avenging his sibling who has burnt just minutes before.
For a good 4-5 months I never played on any other difficulty other than "peaceful" as I was too scared to go through that again.

Lots of things in this game scare me. Any mob that gets the drop on me all unaware. Getting hopelessly lost in a hostile environment. Hard-to-avoid lava (hurry for water buckets!). That my browser's spell-checker doesn't work in these forums.
But my biggest scares are real-life related. I am acutely acrophobic. In other words, I am morbidly afraid of heights. This is as true in Minecraftia as on Earth, the one difference being that while in the real world acrophobia is constance, in-game the symptoms wax and wane, though they are always present to some unpleasant extent. I get dizzy. My arms and legs shake. I become nauseous.
Looking down from the top of a ravine is very difficult for me. I can't do it for any length of time. You rarely see me create ladders to any great height. I instead rely on enclosed (when possible) stairwells.
Case in point; the "homing beacon" situated atop the living quarters of my "hill fort", a one-block wide column of wood with torches along three sides. It isn't all that high, yet I had the hardest time building it. It was supposed to have torches up and down all four sides. But each to I ascended the column to place torches I became more nauseous and my shakes grew worse. By the time I was ready to start in on the fourth and final side I had made myself so sick I simply couldn't take any more. I had to exit the game and go lay down a good thirty minutes before I was able to function again. I never got around to finishing that fourth side.
Now, I don't think this is necessarily a bad thing in-game. Adds a bit of spice to game-play. That's not to say that I don't dearly wish I were not afflicted with Acrophobia. (On the other hand, first-person "head bobbing", which bothers some people to no end, doesn't affect me in the least.)
I sometimes play at work during down times so I keep the game muted. I'm terrified to go in caves or wander at night because I never know if something is creeping up behind me.
Well, I was scared about Herobrine like, 3 or 2 days ago... I didn't really play Singleplayer then. But yesterday, I finally realized that he isn't real, and he never will be. And all those things of Notch saying "he might be in another update" is just Notch trolling. So...
Well I'm not really scared about anything in Minecraft, but I will say I get extremely nervous near lava when I'm down in caves. Especially when I'm carrying diamonds or diamond tools/armor.
I will have to say that in the last few years of gaming, that this game always gives me slight fear of going in dark places. Even after I outfitted myself with armor, and I know I can survive an attack, its whats lurking in the unknown that frightens me. And Creepers, sheesh even in daylight I am always looking 360 because I know soon as I start working, 1 will sneak up on me and wupe out what I am doing.
LOL, and i was litterally hiding in the corner in the edge of my little cave house when i first started playing holding my wooden sword. Seeing a skeleton outside was like seeing chuck norris karate chop you in the face to me.
I won't lie. On several occasions I've been happily mining away only to have a silent zombie sneak up on me and start attacking. I nearly jump out of my skin each and ever time.
Play on a hardcore server with a deathban. I used to play on them all the time, and seriously, there's no safe times. Every minute you play on them is sheer terror. They're a lot of fun :D.
Apart from that, I get scared often if I'm mining with a lot of valuables. Anything can happen.
