I was working on docks, and tried out soul sand- great for stopping boats from breaking, but I kept getting suffocation damage when I got out. This wasn't a problem until I decided to type something to my friend once I landed, and when I got out I died- and all my stuff fell 20 blocks deep into the ocean
I've definitely had the obsidian-mining death, which was especially ridiculous as it was in my own farm, I just pressed W without thinking. My most recent death that I should have seen coming was when I was leading a bunch of horses back, and one got stuck hanging, so I mounted it to try and help it onto land. I KNEW that anything left hanging will take fall damage when it gets on land, and it had been hanging about 3 minutes- I just didn't put these two together until the horse finally found a block and we both immediately died. I was about 10000 blocks from home, but I somehow made it back and recovered the other horses AND all my things!
On the Xbox 360 edition I have a community with like 7 people. Its built into the side of a huge mountain. I have a "lookout tower" at the very top of the tallest spire of the mountain and had just finished building the platform and told my friend "Hey look up here!", no sooner did he look that I accidentally fell off and plummeted to my death. He STILL gives me crap about it months later lol.
I went to the Nether without flint and steel, and when I built a dirt house a ghast destroyed it and before I had a chance to teleport back back it hit the portal so I dug a little hole to protect me and I fell through a cavern into lava with a diamond suit of armor and sword.
Mine was either with I told my sister back when I a n00b, "If I jump, I'll live". There was a big ravine down there with a little bit of one-block deep water. I thought I could jump in any water and not get fall dmg. Just to prove it an have fun, I jumped. I hit the water and died.
Or my most embarissing death was back when I first spawned. This was one of my first deaths. I saw a pig, I punched and punched and punched. I eventually chased it into a little pit with water so it couldn't get out easly. I killed it in there and that was the very first mob I killed. I felt really happy. Later that night the monsters chased me into that very same pit and killed me there.
Dug straight down and fell in the lava at the center of the End portal frame.
I watched my friend do that on his xbox, we just sat there staring at the screen for a few minutes before he threw the controller yelling this world starting over
My worst was while showing a friend how to play, i saw a large ravine and tried to show him the safe way to get to the bottom. I dug one dirt block and fell into a lava pit that I hadn't noticed before, lost a full set of enchanted diamond armor as well as my enchanted diamond sword, pickaxe, and flame enchanted bow. The only thing that held my rage back was the 3/4 stack of diamond at my base
In my first minecraft world, I was holding a lava bucket, and was righting clicking to go to bed, but I wasn't close enough to the bed. So the lava came out, and I was so surprised! Instead of thinking, I just walked right into the lava in fear. I had no house after I re-spawned because I made the house out of wood.
I just had another embarassing death. I noticed a water flow (consistent with a hidey hole in a lake) in the frozen lake near my house. I chopped ice and went down there. As soon as I got out of the water I fell down a little ways and landed a random stone block that saved me from sure death. I built a little platform to make sure i wouldn't fall. I then went back up the water and tried to swim to the surface, but couldn't break the ice in time so I died. level 33 xp. too. I got all my stuff back though.
I saw some current in a river in an extreme hills biome. I went in the current and fell in a 60 block waterfall leading directly to a lava pool. I also had all of my enchanted junk.
I hate my life.

Before you judge a man, walk a mile in his shoes; after that who cares? He's a mile away and you've got his shoes!
I had the AETHER mod and was trying out my vanilla world and fell 3 blocks and lost 33 levels and a full set of iron armor and iron tools and all of my food. I lost all of my stuff for some reason.
Last night I was killed by a baby zombie wielding my own diamond sword which I had dropped after being killed by a zombie horde just a couple of minutes earlier. Oh the irony
Gather 'round & I will tell you a tale of noobyness the likes of which you've never heard.
On my first minecraft world ever, I decided to go for a happy little swim in a small pond just that was outside of my hillside shack. The sun was shining & the water was only about 2-3 blocks deep. A noble pig was keeping watch. All seemed right with the world. What could possibly go wrong with this cheerfull endevor you ask? Well, as I said, this was my very first world on minecraft. I didn't understand that I needed to use space to swim.
I sunk to the bottom of my happy little pond, and drown. Then, thinking it was a bug or glitch that had caused my death, I dove back in to retrieve my stuff... only to drown again. Because "I R smrt", I repeated this suicidal scenario once more before finnaly tabbing out to find out why I was failing so hard.

Tldr; I was repeatedly pwned by a small pond.
