Actually you'd need four Fortune I books, or else two I's and a II. Now, you can't just add another level I book and have it raised to 3. Since the pick is at Fortune II, you'll need to combine it with another Fortune II to raise it to Fortune III. Look at the wiki tables and the notes under them before using the anvil to do anything you are not 100% sure of having done before successfully. Because it will charge you levels to do many impossible combinations as if they were successful, you just don't actually get anything for the levels you spent.

When did you try, and what version do you play? There was a bug where they wouldn't stack up correctly, but I'm pretty sure that was fixed in 1.5.

Well that explains it. II is already the maximum level for Knockback, you can't get any higher than that.
