Most helpful item? I'd say a wood pickaxe. Without it, you wouldn't be able to mine stone, which helps you mine iron, which helps you mine other ores, etc. So without a wood pickaxe, your current world most likely wouldn't be able to be where you are in it right now.
I can't believe dirt hasn't been mentioned yet.
Without dirt, you can't grow trees and get wood, so you can't craft chests, tools, and a lot more stuff.
Without dirt, you can't grow wheat, so you can't make bread, breed animals, and a lot more stuff.
Without dirt, you can't grow sugarcane, so you can't make books, and you can't enchant stuff.
Without dirt... (and the list goes on)

For everyone saying that dirt is a block, it is also technically an item; after all, it can be part of an ItemStack. And how else could it be in an inventory?
