I just had something weird happen to me that in thousands of hours played, has never happened before.
I'm 30+ hours into a 1.9 survival world and it was looking great. I hadn't died once, and I've slept in my bed at my main home dozens and dozens of times. Well I stupidly walked outside my house to kill a zombie that was bugging me but I didn't look behind me until I heard the dreaded "Ssssss".... BANG!! I'm dead
No big deal, I'll respawn 10 blocks away in my house.... Wrong!! I respawned at my original starting spawn, like 2000 blocks away and across an ocean!
This has never happened to me before in years of playing, if you sleep in a bed that becomes your default spawn point... Right??? The only thing I can think of, is that I think (not 100% sure) that the last time I slept in a bed was in a temporary house I made in a cave just before entering a large mineshaft.
Could this be the cause because the temporary house in the cave isn't there any longer, or is it a 1.9 bug???
Arrrh damn it! Never mind. I managed to find the mineshaft I last slept in and yeah, the bed was gone. A few blocks were missing from the wall behind it, so when I left, as I was fighting a few mobs a creeper or two must have blown up and taken the bed with it
Oh well, at least that explains it. I thought it might have been a bug with 1.9 or something that had changed as I haven't play MC since 1.7.2.

Minecraft only keeps track of your most recent spawn position, meaning if you slept in a bed at the cave, and then either took it with you or it got broken by mobs, you'll be booted back to your original spawnpoint. Also, if the bed doesn't have an empty 1x2 place for you to spawn directly next to it, you'll be sent to the starting spawnpoint.
A bed can only successfully reset the player's spawn point if the block at its head is opaque. Beds with transparent blocks at their heads may still be slept in, but dying will cause the player to respawn at the original spawn point with the message "Your home bed was missing or obstructed."

I must disagree with the replies. This problem has happened to me 3 times now, I know for certain the bed was undamaged, I spent a full night sleeping and yet my next death, which wasn't too long after waking, I respawned back at the original point a long long way from the bed. That is actually a 100% failure rate since I returned to the game a couple of weeks ago, so much so that I've really stopped using beds, they seem so unreliable.

Your bed must have been broken. I don't know the layout of the area where the creeper blew up, but he might have destroyed your bed.
If you sleep in a bed, that bed still has to be there in order to respawn next to it.

You also have to check if a bed has a valid spawn point.
If after you sleep you spawn on top of the bed, then you don't have a spawn point. There has to be a clear block on any of the 10 blocks around it.
