I am sure that this has been put somewhere in another thread, but I can't seem to find anything. I am trying to enchant a diamond or gold helmet to Respiration III, and I can not seem to get it to happen. Everything that I read indicates that I should be getting it. I have my level up over 30. I have tried multiple times, and I keep getting things other than Respiration III. I did get a level 6 Respiration I, but nothing else...
When I read, I see that the enchantment mostly appears as a second effect. After several enchantments, I have not had one of them give multiple effects. Am I doing something wrong? I have my table set up, surrounded by bookshelves and everything else that I can see seems to be in place.
I'm guessing you're playing on something earlier than 1.8? Because in 1.8 it's reasonably easy to get what you want: just put it on the table, look if the shown enchantment is what you want, if not adjust your active bookshelves to test other possible levels, and if all else fails enchant something else for a 1-cost and repeat.
Two alternative approaches are to enchant books instead of items and then, if you eventually get a Respiration IIII enchanted book, you can use that on your diamond helmet. The other possibility is to build an auto-fish farm. In no time flat you will have more enhchanted books than you can use.
Enchanting books is a lot less useful in 1.8, since using an enchanted book costs one of your potential repairs on the tool.
A third possibility (most useful in 1.8, but possible in earlier versions too) is to cultivate Librarian villagers until you find one selling a Respiration III book.
Enchanting is down to luck sadly, I had to enchant 20 diamond swords to get one with Looting III, just keep enchanting helmets until you get the enchantments you want.
It was a bit annoying for me when I went over to 1.8.3 from 1.7.4 and the new Enchanting - because the enchantment is fixed til you do somekind of enchantment, its more useful now to have Enchanting tables with adjustable bookcases so you can see enchantments for different levels. My table is built in rather a confined space, so I wasn't able to do much with it, but I still managed to cram in a bookcase changing system, which has proved beneficial for getting lower level enchants of the type you want, that you can combine later.

There is also the alternative of Villager Trading. If you have access to villagers, you could breed them up til you get a librarian that will offer you a Respiration III book. Then you can get one whenever you want.

That's not how things work; you need FOUR Respiration I to get Respiration III, as you must combine enchantments with the same level (Respiration I + Respiration I = Respiration II; repeat for the other two and then combine the two Respiration II books to get Respiration III). This is also an exorbitantly expensive and impractical solution in 1.8 as every single working on the anvil doubles the prior work penalty, until it is too expensive after just 6 workings; you'd have to do three just to get Respiration III, then another to apply it to a helmet).
Remember too that you only spend 3 levels to make a top-level enchantment, so it is also impractical to enchant at anything less.
(also, I actually make enchanting more challenging in one of my mods; I added some fancy new items (only better than diamond in durability, otherwise the same, not worthwhile for most people) - but I also made them unenchantable on the table, only with books, with the result that I mined more than a double chest of quartz blocks (without Fortune) to get the XP required (current score of 60,000) for enchanting; 100 in-game days in and still no dragon yet or even building my main base; on the plus side, that quartz is good for building with and running around mining quartz isn't much different from my normal gameplay. I also enchanted around two stacks of books at 1 level each since I was combining them to get max-level Protection, Efficiency, etc - something which is actually practical to do prior to 1.8 and cheaper than level 30 enchantments unless you are very lucky, and only for Sharpness IV, not Sharpness V, and so on)

I have a chest of unenchanted items which includes a full set of diamond armor, tools, and sword, plus a bow, and stacks of books.
I have a chest of "partially" enchanted items which need to be combined with other items/books to get what I want (for example, Efficiency and Unbreaking)
I have a chest of "fully" enchanted items which are ready-to-use
As soon as I hit level 30 (due to nonlinear XP/level, sooner is better), I go to my enchanting room. I have a loose priority system written down (basically a list of what I need the most). Let's say, like you, I really want a Resp III helmet, but also Feather Falling boots, and, failing that, another Eff/Unbr pick never hurts. So, I put the helmet in first. If it's Resp III, great! If not, I put in the boots. If I don't get Feather Falling, I put in a pick. If still no dice, I put in a book and if the top-level enchant is crummy, I just spend one level and essentially throw the book away (I have a chest connected to hoppers that sends them to a big storage room).
This way, I maximize my resource/XP efficiency while still eventually getting all of the enchantments I want. However, if you're very desperate for Resp III to the exclusion of everything else, then you would just put in a helmet, no? reset, and try again. I'll warn you, this can be frustrating depending on how unlucky you are, so it really pays to plan ahead.

You can get a basic one set up in about half an hour, and then I just park there when I'm watching YouTube videos or whatever else, come back every few minutes and whack a few more mobs. Enchants only cost a few levels, which you can get in a matter of minutes with a good XP farm.
Finally, in the meantime, it's worth mentioning that you can get a breath underwater by placing torches, or building a partially-enclosed overhang you can run to and removing the water source block. Respiration III just buys you time; these other methods allow you to stay under indefinitely.
I usually do that but as it was was actually Looting III (ideally with Sharp IV) and WITHOUT Knockback I was looking for. So when I saw the looting or the sharpness I enchanted the sword and otherwise I put the enchantment on a book as that rerolls it too.

Why do I still play in 1.6.4?
The item frame deal there is kind of neat, but I prefer this design that requires manually placing/removing a few torches that allows for trying all 16 different levels of bookshelving rather than only 8.
Combining low-level enchantments is a losing proposition in 1.8, since you can't get unlimited repairs anymore.
