I play on Peaceful till i make a base then i set it to normal but back to peaceful if i need to go outta my house to collect recourses, explore caves,etc.
Almost always hard, but no mods. Adds that extra element of 'pay attention' without any huge problems, ya know.
Except when hunting for glowstone, then I go peaceful -- just because it's usually meant to make one of my projects prettier, not really taken for the challenge.

I used to play all the time on Peaceful, but now I play on Easy nearly all the time.
About the only time I will put it back on Peaceful is when exploring a new cave system. I would rather not have a surprise encounter with a creeper dropping down on me from above. This almost happened once when mining some gold right at the entrance to a cave. I had just finished mining it and turned around, and was removing torches when I heard the sound of one taking damage from behind me. So I turned and shot it with my bow. It didn't leave behind any gunpowder. Then as I was blocking off the cave entrance from the corridor that opened into it with cobblestones, I heard skeleton sounds, and decided not to bother, especially since I had over a full stack's worth of gold at the time.
Normal, but: in case of BS, break glass (turn it on peaceful)

Hey look buddy, I'm a builder. That means I build custom maps. Not maps like "Escapecraft 3", because that would fall within the purview of your idea of most puzzle maps. I build adventure maps. For instance, how am I going to allow some big old forum user to enjoy adventure to the fullest? The answer? Use a pyramid. And if that don't work, use more pyramid. Like this big, old, dusty pyramid designed by me. Built by me. And you'd best hope... being played by you.

This about sums my Minecrafting experience. I play on Normal constantly, even when building (what's the fun of building without the imminent threat of death?). If I'm about to lose a bunch of valuable items (IE diamond) or if a squadron of Creepers is charging me and I'm stuck against a cliff wall...I have no qualms about swapping out to Peaceful to save my skin.
Although, I'm also a very, very careful Minecrafter. I've not yet died on my SSP world, and that's after about 2 weeks (IRL) of playtime. (And that's on Normal throughout the whole ordeal - never actually switched over to Peaceful)

I was using hunger and 3 zelda hearts with mo creatures/humans
If I died I deleted the world and restarted
But now I like my current world too much
