Do you wander around aimlessly looking for a good spot to build? Go into creative to fly around for a spot? Stay were you spawned, build a small Shack, and then terraform the area around to suite your needs?
Personally, I wander around until I find a spot. I usually end up in snow type biomes, or deserts that stretch on for miles when I first spawn into a world. So I try to find a small little area with a small stretch of flat surface to build a small farm, and a Shack, then start to terraform.
I almost always stay in my initial spot for a few MC days collecting basic tools, food, and materials. I then spend a very long time as a nomad until I've discovered a place that I deem adequate enough to set up camp in.
1. Find an area for whatever my current build idea requires
2. Make a temporary base nearby. My fave design for this takes the form of a platform built on a single post about 15 min blocks high (or on top of a big tree perhaps) with ladder access - this design is pretty mob proof (inc from pesky spiders) and here I store stuff and is basically my base of operations.
3. Then its basically do some mining for resources, acquire plenty of wood and saplings to grow trees nearby if need be, get some wheat growing and locate some cows to breed up the numbers. I also usually use this time to think a bit more about my build, and usually takes me quite a while before i commit myself.
I like to build by spawn if it's a nice area with a decent spot to build, but spawn is generaly some clustered, crappy, hill filled area. I usually end up going 1000 blocks away or something to go live in a nearby cave while I build.
I've been playing for about 2 and a half years and I've only really started 3 times.
Very end of Beta 1.8.1, my first time playing. I dug a hidey hole into a cave next so spawn, then build a house a short distance away. Didn't play on this world for very long.
Started a server with my friends in 1.0.0 and dug a hidey hole near spawn, explored a decent radius from spawn, and found an island to build on. Lived there for about 2 years.
When 1.7 came out with the new biomes, we explored far away and made a new "center" of the world. We explored quite a big radius around this new area and I have began building a new town on the top of an extreme hilled savanna.
I usually grab some wood, make a crafting table, tools and a sword and wander, wander, wander. I just cannot stop and set up where i spawn without wandering like crazy. I have to know what's around. Sometimes I just go back to spawn and set up there, but not without a good idea of the nearby terrain.
1.- Look for a good seed in Amidst until I found one with all biomes nearly.
2.- Play for a couple of hours at spawn
3.- Delete world.
4.- Repeat from number 1.

2. Build a simple shelter and make my first set of essential tools.
3. Dig down to bedrock and collect ores.
4. Once supplied nicely I head off to find a nice spot for a larger main base.
5. I like to find a place near a few different types of biomes so I have access to multiple supplies.
I set up a base near spawn and then smaller settlements throughout the world as I venture out and discover new and interesting places. I tend to turn every village I find into a well lit, fenced spawn-safe zone and build new houses in the same style as the rest of the village until a natural iron golem starts spawning. My main village residence has 3 iron golems though.
I spend a lot of time underground sucking all the resources out of the cave systems. My desire is to light up every single cave system I can find either by seeing or hearing, so you can guess that I need a pretty big and flexible storage facility/ies. My favourite thing to do in the game is exploring and cleaning out an abandoned mineshaft, that is, taking all the random blocks from the ceiling, all planks and fences as well as ores and cobwebs until all the tunnels are perfect 3x3. Those fences come in handy since I usually need several stacks of them for each village I find.
I usually punch some trees (as do we all) and then proceed to find some Stone blocks to make a Furnace. Once I've gotten the materials I need, I usually either build a cave into the side of a mountain (usually the place with Stone) or use a bunch of dirt and just build a tower under me and wait out the night. The next day I get a whole bunch of resources and then find a suitable place to stay.
I've always built right at spawn, which must be in a plains biome with a large flat area of around 100x100, mainly because my first main world was like this and I've built my base the same way in every world since then, down to building a village in lieu of the naturally generated one in my first world (even though I don't trade with them).
I also did make secondary bases, linked to my main base with a rail system, in my first main world, but since then it hasn't been worthwhile to do so since back then I didn't carry an Ender chest (much less one modded to have 54 slots, or a backpack mod I used for a while) while caving and only used unenchanted iron tools for the first couple months (aside from a Fortune pickaxe for diamonds) and only mined what coal I needed until 1.6 added coal blocks (coal is currently around 2/3 of all ores I mine), so it took a while to fill about a double chest with ore blocks and loot (filling up an Ender chest and my inventory for return trips to drop it off at my main base), making such secondary bases practical (each one used over several weeks).
I used to stay pretty close to the spawn area. These days I'll usually rove around right after spawn then maybe settle down when I find a nice area to set up my base of operations. I only jump into creative once in a blue moon to test something out in a different game, or to set up things on one of my servers. Other than that, I'll pretty much just play in survival hardcore.

I generally build a shelter with a little farm and then dig down to diamond level from there. I generally enlarge my first base over time but it tends to be haphazard, although they've been improving over time. Once I'm well established, I start spreading out and building more carefully planned bases, usually near villages or inspirational scenery.
