If we're going for realism, then whether or not you harvest them with a tool they should hurt you when you pick them up unless you pick the cactus pieces up with tongs.
I got a few cactus with me right now, I think I could pick them up without hurting myself. Anyway, taking damage from a cactus seems trivial.
1. Would you be able to bring yourself to punch a cactus in real life?
2. Cactus hurt, but there is no way that they could kill.
Only an idiot or a neurologically impaired person would stab himself with a cactus. i grow barrel cacti in my house [got 8 big ones in my yard now] and you can pluck them right out of the ground if you watch where your hands are.

I think of these more as the furry-looking ones that hurt you immensely if you touch them.
That being said, I don't think you should be hurt because of you placing blocks on them. That makes literally no sense.
I agree on the building onto cacti part, you should be able to add stuff to them without hurting yourself. Using any tool should be able to prevent you from sticking yourself since you're not directly touching it... Harvesting cacti without tools should probably be impossible, like someone else suggested. Then again, you can also punch through rock in minecraft, so the player character must have some very tough hands

in real life a cactus would probably be harvested with a machete or a knife depending on it's size. so a sword or a shale knife would be preferable
