If you die and you have this item in your inv you will resurrect without losing any items or exp. You will also not treasure any damage for the 1st 5 secs after you resurrect, so you have time to escape/get out of lava. Sounds alittle over powered right? The recipe will take time and exp.
First you would have to get yourself blaze powder and a feather (recipe will either be combine the 2 or surround the feather with 8 blaze powder). This will make 1 blaze feather which could possibly have more purpose later(flame arrows?)
Next you take the blaze feather to an enchanting table to get a lvl 10 lvl 25 enchant on it (only enchant allowed). This will make the feather into a phoenix feather.
Maybe have different lvl of phoenix enchants such as 25%, 50%, 75% and 100% health/exp back.
Edited: The feather will give you back half your hp and leave you hungry. This means you cant continue to fight, but will have to retreat and eat to get your hunger/hp back up.
Making the item into a buff is one suggestion which I like better. I believe the buff should last 10 minutes or vary depending on the enchant level. What do you think?
This item since its unique will not stack, nor will the blaze feathers stack to make enchanting them easier. Since they dont stack each feather will take up space, meaning the more you have, the less you can carry.
A phoenix feather should be crafted using a feather, 2 ender pearls, 2 blaze rods, and 4 pieces of coal/charcoal. It should have the ability to ressurect you when you die at the exact location you died, even if it was in lava, as opposed to going to the death screen. You should lose all your expirience, but keep all your items. When you ressurect, you will have similar effects as when eating a magic golden apple.
I agree with the option to choose if you want to resurrect, but I feel it can be done without that option as well. Of course if you do resurrect you would lose the feather. The idea of not taking damage is to prevent resurrecting just to get shot or burned before you can move. I believe the enchanting would make it something that you can't just stock up on because once you have your chicken farm and found a blaze spawner, you could create them on the cheap but with enchanting it would take not only materials, but exp meaning you would have to prepare ahead of time.
An aternative, might be a trade with a villager, feather + 20 emerald = phoenix feather.

At first I would agree, but looking at the requirements for the feather I feel that its pretty balanced. Unless you have creative mode I feel this would take a few hours or maybe days before someone could get their hands on this.
When I put "never die again" this would require alot of these feathers. I would have included a phoenix bird to be added to this (probably found in the nether) but I feel that it would be a bad idea for another flying nether mob and how do you kill a bird that gets reborn when it dies?

They would be consumed at the time of death. Maybe making it a lvl 25 enchant would make it harder to get and therefore less people would use them. Its basically a safety net. Making it a buff might work too. I do like that idea, but it should last for 10 minutes to pass the night cycle. If you do resurrect it could possibly put you at half health and no food so you can't exactly continue fighting, but would have to use the time to take escape whatever caused your death and heal up.

To me one of the most tense parts of minecraft is when your deep in a cave system or the nether with a lot of valuable stuff and you might die and have no way of getting it back. It's what makes minecraft kind of nerveracking and with this item it just makes it so the player doesn't have to be careful. Therefore I think it's a bad idea

Instead of making it carft-able, make it tradeable. In limited location and in limited number.
Problem with tradeable is that the villagers currently seem to have unlimited of what you want, as long as you have the money. I also believe that anything that you can buy in the game you should be able to make or gather naturally. This is why I dont play most games with crafting. In most games the item you can buy is better than any that you could ever craft. In this game I think its important to have any item able to be crafted or found.
As for the item being over powered, I could agree with that, but the amount of work that most players would have to go through to enchant a feather like that would make this item limited. Unless a person cheats I believe most people would keep 1 or 2 at most, since it may seem like a waste of an enchant. Plus with the needing to go to a nether fortess to find the stuff to make it, this might take even for time.
I do have 1 idea to make this even harder to do...
What if there was a special enchanting alter for this to take place and you can only find this alter in the nether fortress. When you break it, the alter will dieappear, unless you use silk touch.
Diamond tools are over powered in a way, but you must find the diamonds. This pheonix feather is over powered, but you must go through alot to get it. If it was a trade item then I might agree its too powerful.

Minecraft is already easy. And this feather takes all the challenge out of hardcore. Maybe it would be okay for adventure mode as a form of extra Lives.

Its based on phoenix feathers from multiple rpgs, including Final Fantasy. It does sort of take the sport off of hardcore, and would be excellent in adventure mode as a form of lives. I don't believe it would be easy to make this item, although I guess for some(not most) they will probably get this easy by building monster farmers. I feel the main thing this item would make easier is the end dragon. If this item is ued as a buff it would probably be used less because it would cost too much to keep the buff going.
Maybe this phoenix feather would work good as a mod and then if it works change into a feature.

I like that idea, even if you did revive after falling into lava, most of those items would burn. As for after a fight it'll only give a chance to run and maybe regroup.

I'll be honest, I really like the idea. I also like the fact that you actually took some effort to mention balance. I thought this was thread was just gonna be a "you can make this feather and use it to never die! please support!" post without any actual detail, but it's much better than that.
On to the downside. Though I like the idea, I think putting this in the game wouldn't work out too well in the long run. I say that because we spend a lot of our time stockpiling on resources to give ourselves good weapons and armor (and even potions) to not die in the first place, so this idea would just be used as a "get out of death jail free" card. Players would find ways to exploit this Phoenix Feather and spoil the difficulty of the game.

If these "phoenix feathers" were to be consumable(like food/potions) to give a buff of invulnerablity.
I think any method of stopping lava deaths other than skill, armour, potions, or water is just silly and overpowered.
