a guy known as Dan4548 just suggested to me that its gaze attack should void the protective effects of armor, because your armor will not shield you from your mental state.

I LURVE the hallucinations idea. the way you could tell that the mob isnt real would be either. its slightly dis-coloured like maybe a skeleton would be slightly more grey. and they wont have the same behavior. so a skeleton will just run at you and eventually straight through you instead of dodging and shooting arrows.
i honestly dont like the hurting idea. but it could make you THINK your being hurt? so the player goes "UHH" and the health bar goes red and all that but no hearts get taken. then it makes you think your on fire. makes it look like there are arrows flying at you from random directions. try and make the player move or look away. THEN if none of that works it should make a loud noise and teleport somewhere else randomly around you that is the exact same distance away from you plus 5. so you cant hold the gase forever but you still have a chance when it moves.

I like this idea, the 'trying to get you to look away' thing would naturally seem like something they would do if they couldn't move while you looked.

well, the idea of it hurting you comes from the concept that you are just slowly losing it! insanity!
but, now that im thinking of it, i like DoomFire's idea more. you would never really be in any danger at all the whole time (unless a real mob attacked you! and you'd still be kinda dazed after looking away, so now you have 3 problems to worry about!)

What's the point of hallucinations if you know they don't do anything?
It's like that Sly Cooper game with 2 player vs (only example I could think of). I always managed to use that confusion thing on my friend, but it didn't matter because it had an easy pattern. All the controls were reversed and didn't hurt him at all. It has no point.

well, at night, you may mistake the illusions as real mobs! or vice-verse, you may think that a real mob is actually an illusion!
as i said before, i think that what you see in the hallucinations should be dependent upon the time of day - there's no Zombies during the day. (remember, the illusions are happening at the same time as the acid trip you're going through)
i think that the distortion progression should follow this pattern:
light distortions
strong distortions (true acid trip!)
mob hallucinations
strong/physical hallucinations (being hurt, random arrows, being on fire, etc)

maybe these hallucinations could even disguise friendly mobs as hostile ones, and vice-verse. you have to remember, the Enderman is going to do everything it can to stop you from looking at it (im now agreeing with the idea that they shouldnt hurt you from their gaze). the hallucinations may be fake, but you gotta give the Enderman points for trying.

The idea is that the illusions cause you to turn away and let the endermen attack, it's true you'll expect the illusions at some point, but you either have to endure the annoyance or fight a really tough mob. otherwise you could just walk up to the enderman and kill him while he cant do anything.

And that's just playing it safe. While you would be out collecting at night doesn't make sense to begin with. I doubt seeing fake mobs would change anything.

well, i dont care what order it goes in, as long as staring at it gets progressively worse. so it will always begin with slight distortions to tell you that you've made eye contact with it. then it gets worse and worse until it either runs off or it attacks you (or something)

Of course staring at something would hurt you. If you look at it, you would be poisoned! I call ********.
i think that looking at it too long should hurt you, but it would have to be after an extended period of time.

because if it cant confuse you into breaking the gase then it should teleport to break it. but it will always teleport a fair distance away from you.
either that or it should give up and teleport away until you stair at it again. that way you might have some chance to get away from it.
or it might randomly pick one of the two options? that way when it teleports you dont know weather your in danger or not.
