It would be so much better if there was a gamemode that had dehydration, medical supplies to heal instead of food, sicknesses so if you get a cold you have slowness amd hunger for a day
The medical supplies idea may need revisions. Like there would have to be a way to create basic medicines from plants and stuff because when you first start, what would happen if you get hurt.

In general, most people here (including me) dislike the idea of sickness and thirst because it adds tedious micro-management. Going into this suggestion I was expecting to argue this, but there's a far bigger problem here.
You have offered zero detail on how such a widely disliked idea could be added. If you have put lots of detail forward, and give strong reasons on why to support this, I might have supported. But right how you're just going off the assumption that everyone agrees with you already, which they probably won't.
And no idea farming. Come up with this yourself, then post it to the community.
Don't support.. BUT I do like to idea of having more 'realistic' effects on players ingame, perhaps something like a slight slowdown in walking/running when walking through snow, perhaps your health could decrease that 'little' bit faster in cold climates

It would be so much better if there was a gamemode that had pointless micro-management, gamemode-exclusive items (which already exist in the form of potions, by the way) and annoying effects that you have to deal with depending on the Random Generator's mood.
Do you even know what a gamemode is? A gamemode is like Survival, Creative, Adventure or Spectator mode. (Hardcore is actually just survival mode, but with death being handled differently.)
I don't want to see a gamemode that is litterally just Survival mode, but with extra gameplay added on. That's not what gamemodes are for.

I'm very alone in my Minecraft worlds as I don't have a very good internet connection to run a server. If you're like me, you might be interested in my Posse mod suggestion.
Ideas like these make cool mods, but poor vanilla mechanics, because, as has been said, they're inconvenient. One bar to micromanage is okay, and the hunger bar does have a purpose, and food items are ubiquitous enough that keeping it topped up isn't much of a chore. Constantly finding plants to stamp down random debuffs or hunting for glass to store water aren't everyone's cup of tea, and would, in fact, be invasive and annoying, forcing the player to potentially carry unstackable items or craft elaborate recipes just so they don't die in a cave somewhere because they didn't eat enough.
Some people would enjoy this challenge, but others would find it to detract from the simpler aspects of the game. Making it a gamemode is avoiding the issue; it wouldn't fit with the others.
Perhaps, if you added a little more detail, you could post this in the Mod Suggestions forum? It wouldn't fit much with the original game, but I would definitely install it as a mod.
