Potion of Suffocation? What is it? and what does it do?!:
Potions of suffocation and the opposite of water breathing potions. They cause the player to slowly suffocate on land, and will speed up their suffocation while in water.
And what's that useful for..?
Everything! From PvE to PvP! How does it work? Easy, after 10 seconds, the affected mob or player starts "drowning" inland. It is dealt 2 hearts of damage per second. This will be the "put and forget" potion. Basically this puts pressure on the player and insures them high damage after 10 seconds. Balancing it out, high damage but it takes a long time to start the effect, leaving the player with enough time to escape from combat, eat food to regenerate or just drink a milk bucket to cure the effect.
How to make it?

Normal type ( 13 seconds ), obtained by using a fermented spider eye with a normal water breathing potion.

Secondly, because it makes timing a lot more important in combat. It adds that feeling of danger where you might die at any second because of this debuff. I feel like that's exaggerated, when in PvP, there is always a feeling of danger, when going up against a stronger player or a band of them. Timing is also already an important aspect in combat, swinging too fast deals less damage, and doing it correctly gives a direct hit. I don't see how throwing potions makes timing important, because that aspect would have already been taking into account when potions were first added. This potion is no different from any other.
Thirdly, it's high damage is balanced by it's long duration. But unless we know how much damage it actually does, we can't deem if it's balanced yet.
And fourthly, because there are very few negative effect potions. Fermented spider eyes lack good uses as the only negative potion that can be used in combat is the harming potion, and that doesn't work on undead mobs. Because everyone knows that Slowness, Poison, and Weaknesss are useless. Slowness slows down your opponent, which is helpful as an escape tactic. Poison helps brings down your opponenet's health down, and Weakness lowers the opponent's attack damage. The Fermented Spider Eye is anything but useless, and it's use is by fair greatly helpful in creating these potions, just as useful as using Glowstone or Redstone in making potions.
Responses in bold. This suggestion has some potential, but it is missing some information and areas that are flawed that is listed above.

Also take into consideration that your opponent will also be doing damage along with this. So if this potion won't kill you, the number of hearts you have left and your opponent present will most likely will.The enemy will have 10 full seconds to prepare himself for the damage, it isn't spiky like poison or instant like harming.
Well what are other counters? As far as I can tell, that's only two, and I just pointed out the problem with this counter. Besides milk, running away and just plain health regeneration, helmets with water breathing enchants work to slow the suffocation, if not end it before they even start taking damage. And by the same logic, potions of harming have ZERO counters, armor doesn't help because they're potions, they instantly deal damage, milk doesn't work.
But still, they make a great offence, and they technically do give off a negative effect and are an opposite of the Regeneration Potion. I agree with you on this but they still don't give fermented spider eyes a use because they don't even use them.
I'm a little confused, can you give me an example? For example, the elytra that just got added in 1.9. Can it be used in DIRECT combat? In a sense that it deals damage? No, it's very situation. The mobility it gives is rarely useful considering the great sacrifice made by not using a chestplate.

You misunderstand him here.
For one, he's not saying 2 hearts of damage, but two HEALTH, equal to one heart. He's basically saying that the potion will make you drown like in water, but above ground.
Second, when he says "long duration" he means the 10 seconds before the drowning starts, where you could counter with milk, a water breathing potion, a helmet with Respiration, or whatever else.
Now I'd like to deliver my verdict.
This seems like a neat idea, but I'd like to offer a few suggestions in your different areas.
It makes you drown above ground and has quite a few counters. That's neat, but if being in water accelerates the effect, that's a bit counter-intuitive and OP in my opinion.
What I'm suggesting is that water isn't something to avoid while you have the effect, but yet another counter.
While in water, you can breathe freely. The potion could be named "Gills Potion" or something like that.
Besides that, the counters of water breathing items and potions are neat, and milk is always there if you don't have that.
Although I do believe that the duration should be nerfed to last a little bit shorter. It is a bit OP.
Maybe this wouldn't fit well in PvP though. Now what a nice idea would be is if a naturally spawning mob had an attack that gave you this effect...
But this is for PvP and I'm getting off-track with all my suggestions... All-in-all, I give it...

The other, using the potion, will need to calculate the time it takes for help to arrive. Basically, all the potions in Minecraft are guaranteed to cause their effect, while this one is a high risk/high reward potion.

I'm not the biggest fan of this, because besides what other people have said, you claim this has a use in PvE and I'm really not seeing it. If I have basically any weapon in the game besides my fists I can kill just about any non-boss hostile mob before this potion would start to cause damage to it. In fact due to the range of a splash potion and it's area of effect I'm probably more in danger from it than they are if I get caught in the splash.

They don't swim down and even if they fall from the top of the world into water they don't sink far enough to start drowning with this before they surface. If I have basically any weapon in the game besides my fists I can kill just about any non-boss hostile mob before this potion would start to cause damage to it. And this adds to the fact it is has a high risk but also a high reward.But it also makes it kind of pointless. Why would I waste the resources to make this potion (meaning 1 glass bottle, 1/20 of a Blaze Powder, 1 Puffer Fish, 1 Nether Wart, 1 Gunpowder, and 1 Fermented Spider Eye) when I could kill anything that this would do a reasonable amount of damage to with a Wooden Sword in less time? Heck I might be able to kill it with my bare hands before the potion would kill it if I get all critical hits.

Except that poison doesn't really kill, unlike this potion.
Sure, it can't kill you, but it brings you down so low a punch will.
