You know the holes that are sometimes beside a lake. The sand that hadn't fallen yet was my spawn location. On the first spawn it all collapsed. When I tried to respawn after dying I just went strait down and at the bottom dead again before I could even see anything. I would have had to edit the map and changed the spawn location to even explore the map. Never did just created a new map.
Almost every seed I've ever made: a simple forest and plains. In case its not obvious, I hate those. I want exotic things, like Savanna M's and Jungles and massive Desert hills.

Download Amidst 4.0 (see my signature), run it, click the icon in the lower right corner and select the biomes you wish to highlight, and start hitting Ctrl-R until you find a world you like.
I suggest that you place Amidst in its own folder under the .minecraft folder and create a blank text file called history.txt in that folder.

I've had several seeds where I start out underwater. A couple of them I was in Deep Ocean and drowned before I could get to the surface....... Aside from that playing with large biomes I started out in a desert and died before I could get out of it to find any trees. That's the reason I like the bonus chest as it gives you a fighting chance since that started.

Rather than re-create the world with bonus chest turned on why not just set game level to "Peaceful" until you get somewhere livable?

I could certainly do that but I was trying to see if I could make it.... Didn't work. Also usually by the time I think about switching to Peaceful is just as I get that "You Died!" screen. If the seed looks really cool I may restart and go Peaceful and then explore. Or go Creative and build a supply stash then switch to Survival and go from there. I play with a LOT of seeds so I've had the whole range from dying soon to spawning in right on top of awesome stuff. First time I saw an igloo in the snapshots I had spawned right next to it. Interesting thing about the seeds where I've spawned under water in the ocean - usually it's quite near an Ocean Monument.
I've also been known to just tweak the world generation too. I love those settings. I had one seed where I started out survival BUT there was a really cool hill that looked perfect to build a castle on and a village nearby. So I did a new world with the same seed only tweaked it a bit and went full Creative and built a nice castle and extended the walls around the village then expanded the village - with my son's help
Created world with lava oceans, bumped up the ocean level a bit. Ended up spawning in island, that was at the same level as the lava ocean was. And that was only block that I could stand on, as there wasn't other blocks.
And I did have screenshot from that, but that was at summer and my computer got pretty much destroyed by power surge. Thankfully got it repaired and majority of my stuff saved, but lost my main world back then, and everything from my .minecraft folder.
You spawn on a small island in a pool of lava, so you die as soon as the world generates.
However, I do actually like the world because there is an ocean monument right beside this small island. There is another island within swimming distance that has a couple cows and some trees, so you can succeed if you start in creative then switch to survival.

Worst spawn was on a 3*3 island with nothing but sand, even the bonus chest said gtfo (it spawned in the water somehow) so I couldn't even make a boat to leave.

Saw a skeleton murder a spider, but I think it was just trying to shoot me

I've spawned under water a number of times and usually that means the bonus chest is nowhere to be found. I assume it spawned under water as well and just came apart. Went into creative mode but still couldn't see any sign of the bonus chest.
I don't remember what it was, but it had absolutely nothing going for it. It took me hours just to find iron ore. I'm gonna assume there was some sort of generation error. That's the only way my brain could be at peace with that.

What was the worst seed you ever had?
I once got the RNG (no input from me) throw a huge ice-plains island at me. Deleted that world on the first day.
I once had a tiny snow island filled with wolves.

I once got a spawn on a floating stone platform over a ravine. I wasn't able to do anything other than die from fall damage. XD
Also, like one person above, I've gotten multiple underwater spawns too. But the ravine one was by far the worst.

Once I spawned directly above a lake of lava. But it wasn't that bad once I survived the burns, I had a portal in day 3!
