Hi, so i was waiting for the zombies to spawn near a XP zombie farm, AFK and auto-fishing. When I returned (10-15 mins later), I was "slain by zombies".
-There was 3-4 blocks away from where the zombies come falling, and only their feet were showing
[edit: 0.5 horizontal block of open space, between fence and glass block above. See screenshot.]
-No way out of the room, but no Zombies out of the cage when I respawned (same room, same spot - I died on the bed)
-No sunlight/armor/blocks to damage the zombies, and the cage still had a good space. Some DOES die from fall.
Does anyone have an idea of what happened?
This is in 1.12.2 vanilla, java, multiplayer but only I was connecting. The server runs on the same PC. No logs other than log-in.
Questions, wild guesses, any ideas to solve this mystery are welcome.

1:I made the same system simplified in Creative, only the climb and the fall is longer.
2:I also tried spawning babies directly on top of the hopper. They still seem to not get through.
3:The actual room in Survival mode. The bed and the fisher is showing.
Chicken jockey? Those often get past baby-filters. The chicken could've died at some point after it, leaving the baby zombie theory again.
Depends what you mean about "their feet shows is behind a wooden fence" - it's hard to visualise the room layout. Maybe you can post screenshots, or a world download so we can have a poke around.
I'm sure there will be a logical explanation.
ah yes the 25 mob limit must have killed the chicken and baby zombies are smaller so i think they could get pushed between the fences edges if it doesnt connect.
-edit- i tested it out and indeed a baby zombie can and will get pushed between the fence if youre using a setup like this

I made a sample in creative mode to see what happens, but no babies nor jockeys seemed to go past the filter, and when I tried spawning babies past the filter, they still seemed to get trapped (see SS). Plus, note if somehow a zombie got out, they also had to get back in, or despawned for some reason.

Even if some zombies got out, there still remains a question: where did the killer go? They had to either go back in or despawned, or damaged by something and killed.
Entity collision is a thing again as of 1.9, it was probably a baby zombie that got pushed up onto the fence and to freedom. Fences are only 1.5 blocks high and are considered transparent, so it wouldn't be much of a barrier in this regard and even less so for baby zombies. Add in the glass above and there's really nothing to keep them in.
Now, this might not matter so much in terms of entity collision forcing mobs out, but one easy thing you could try doing is to put two pairs of sideways-facing stairs pointing at each other. This would form a gap between their upper stairs that is equal in size to a slab, but it's centered at the blockspace boundary and therefore will cause mobs to not see it as a valid way to you (sort of the same idea that tricks mobs to walk off an open trapdoor). Ranged attacks (and the player's sword attack) can still occur through the gap.
This has always been a thing, it was just removed for players (and only players, which could still push other entities) between 1.3(?)-1.8 - I've seen mobs glitch through fences myself in 1.6.4 due to the same thing (fences also have issues with their collision boxes, namely, all fences must share the same collision box regardless of their state since they are all really one block. This may finally be resolved in 1.13 since every single blockstate will have its own unique block ID).

The babies are killed before they are sent out of the spawner room, and also where their feet shows is behind a wooden fence.

the baby zombie wasnt there the first time because most of the time the one that kills you gets despawned.
