My creative worlds are usually just "test World 1" or "New World/Temporary" I usually delete them, save one with I putz around on now and then.
Modded worlds are usually some type of Challenge, or the main mod I'm playing with there like 'Biomes O' Plenty"
My survival worlds are the only ones with true though. My current one is "World of Blocks", sheerly because there is no main biome in which I inhabit there. My old one, "Desert sands" mainly was desert, with all the other Biomes in near reach. My first world was Creeper Land, seeing as it had a higher spawn rate of creepers (I still haven't seen that many creepers at once in one world)

Sometimes I name them SDF or GYF or some other random 3 letters. When I am weird I name them Prepare for the Fall or something like that. Sometimes just a random emoticon. Sometimes yep or maybe or something like that. I am truly creative with these.
I just named my first main world World1 and the second one World1v2 (exactly the same, except for what I did to caves); for test worlds (in MCP) I just name them Test1, Test2, ..., deleting them when I get to Test9 (keeping one so I can recreate with the same seed); or for trying out snapshots/versions, 13w41b, World_172, etc. I'll use more specific names if I am playing a map that I downloaded (already named) or am playing for a specific purpose (such as Survival Island).
I name them with the seed I use to create them. I usually use short geographical names. That way I can easily tell people the seed and often remember if I'm away from home and want to show somebody the seed.

Since I do the Youtube thing, I try and name them in a way I know which world coincides with each of my series. If it's a personal world, I just stick with the default mostly

Ever since I found out that if you use the name of an old world that parts of the old world might be in your new world, I make sure that I never use the same name twice. I'll use the date that I made the world, then a decimal and a letter to signify which number it is out of the ones I've made that day. For instance, if I made multiple worlds today, the third would be named: 11-1-2013.c .
P.S. While I'm about 99.99% sure that I will never even come close to making 26 worlds in a single day, if I DID happen to go over 26, I would start doing letter combinations. For instance, the 27th world today would be named: 11-1-2013.aa and the 28th would be 11-1-2013.ab .
I've never had the problem with deleted worlds not fully deleting, but I think it is because it requires specific circumstances to happen, like deleting a world you just played then making a new one with the same name without closing Minecraft (i.e. the corruption occurs in memory), or having the save opened up in another program (can't delete files in use); when experimenting in MCP, I've gone through a couple dozen test worlds in one day, starting over at Test1 when I reach Test9, deleting 1-8 at that point, then 9 after recreating it (I use the same seed on all of them, but the cave generation is different in each one; Unmined would show any chunk discontinuities). Even when I left Unmined open when deleting worlds, which causes a deletion error (folder/files in use), it still deleted the region files, just not the files that Unmined creates.
I have a habit of giving my hardcore worlds trash-talky sort of names. "You're going to fail", "You're going to fail again" "Seriously, stop trying", "This is embarrassing", "Why do you keep doing this?". I'm currently playing a hardcore world named "About to die". It seems fitting.
