I recently found myself wondering the same question. Immediately, I believed the dragon egg was the rarest. However, I come to realize that rarity should be seen as a function of use. For example, a ratio can be created- Potential to gather in 10minutes: Recipes/function. Obviously, this is highly situation based, so I assumed a optimal setting for the 10 minute time period. For example, if we are looking at diamonds, the 10 minutes takes place in a lava-level cave. The lower the number, the higher the rarity.
Here are some ratios for various items in the game:
Ghast tear- 1:1 ratio (1)
Wood: 128:26 (4.92)
Diamond: 4:13 (.307)
Dragon egg: 1:1 (1)
Emerald: 7:48 (.145)
From this data, it can be seen that the rarest item that I calculated is Emerald. It is important to note that for the left side of the ratio, this is an average of mining/trading. The right side of the ratio includes trading and emerald block. I agree with the value produced in this equation, because emerald does have a high use, and a high rarity (unless exploited, which was not taken into account). What do you think of this ratio system?

For me, the rarest item in Minecraft is the " UltraMegaSuper Diamond of Awesomeness! " , well, that's my opinion. With that item, you can make the most powerful equipment in the whole Minecraft World! Muahahahah! Muhahahaha! Hahahaha! >

those can only be obtained by killing a wither skeleton, a rare mob with a 5% chance of dropping its skull
all that on top of 3 obsidian and 5 glass
oh and did i mention a mound of diamond blocks for it to actually work

I think it is pink sheep because they only have a 0.164% chance of spawning and I found one today.
