The source of the problem comes from the "How to set up a minecraft server" topic. Which was instructing people to turn verify-names off by default. I have corrected this mistake so future servers shouldn't have this problem unless the connection issue arises in which case they would have to turn it off in order to connect.
I take this moment to say TURN IT ON IF YOU HAVEN'T ALREADY. It's a massive vulnerability which could result in your server getting a bad name and taken over by rogue OPs pretending to be an existing - and trustworthy - OP. Other side effects are members being framed for griefing (See ONI's case), racism (mail's case) or other malicious actions they didn't do.
Also, if you see Notch on a server. It's probably not him. You can check on the IRC to confirm.
All that matters is a whole lot of people need to turn on name verification, and that anything that happens in a verify-names = false server cannot be trusted.

I have often entered a server and if I sense that it may have been set up following the guide in this forum, I ask if verify-names is set to true or false. If they say false or they don't know I use an example to show that there is a risk.

You mean is it possible to connect with it ON because sometimes people have a problem connecting when it's turned on. In which case you should exhaust every other possibility of not being able to connect before turning it off.
Anyway, it IS possible to connect locally with it turned on, I do all the time. So yeah.

Not on the same box. I've never tried it, but I've helped set up 10s of servers, and this is the virtually universal experience.
Connecting to the server via a client on the same machine works perfectly fine for me - just keep in mind that you need to use the external link, not the play.jsp?ip=x.x.x.x&port=xxxxx
