I've tried multiple different ways to keep them in their pen and no matter what I do, I always come back and find I've lost multiple cows. First I tried just the regular fence. Obviously didn't work. Then tried stacking two fences on top of each other to make them higher. Didn't work. I surrounded that double fence with another fence two squares away from that fence so hopefully I could still catch the cows that got out of that fence. Didn't work. I dug a one square deep pit surrounded by the fence. Didn't work. Two square deep pit. It didn't work and I even lost more than I'd ever lost in that small amount of time. I had 14 and I have 8 now. Just in one in-game day. I'm really, really getting sick of this. None of my other animals escape and they're just in regular, one fence pens. Does anyone have any suggestions on how to keep them contained until Minecraft (hopefully) fixes this?
Man, I went mining and left them for a while and there's not a single cow in the pen. And to the guy suggesting using blocks, I did try that at one point, I built this giant barn and they kept escaping too. None of my other animals have escaped at all, it's just the cows. And I'm not sure if I'm on 1.8.1 or 1.8, not sure how to check that.
I think 20x20 is the max amount of space the pen can contain or they are despawning
Nothing you said makes sense. Animals don't despawn.
OP: Make sure your pens aren't too crowded.
If that doesn't work try making the pens two blocks larger around then putting a row of wooden pressure plates on the inside of the fence two block away from it all the way around. Then place water source blocks all the way around next to the fence except in the corner so that water flows inward toward the pressure plates. This will keep the cows away from the fence when the chunks or world load.

That's interesting... Are you sure the cows were fully blocked in without using fences or anything else, just the full blocks?
Animals shouldn't be able to phase through full blocks unless massively overcrowded(like 100 cows in a 2x2 area) but they're pretty glitchy at times(they may have suffocated INSIDE the full blocks) and I'm not familiar with the PS4 version so I'm sure it's totally possible.
The answer to your problem should be a 3 block deep pit. I've never heard of anything ever escaping a 3 block pit. If you only make it 2 blocks deep however, the animals can sometimes get pushed up and out of the pit.
rodabon's suggestion about the water above should also work.

This forum has made me decide that I now want kids, so that when they get old enough, I can forbid them from coming here. it's a terrible place.

-Pixel Format Not Accelerated Problems in Windows 10 Reasons-
1.Old Intel CPU (2nd Gen and older) and these aren't ready for Windows 10.Please downgrade to an older version
2-Slow/Intel GPU (But sometimes Intel GPU just work fine from 3rd Gen CPU or newer on Win10)
3-Update Drivers (recommended for Dedicated GPU but not always for Intel's)
[This signature isn't to critisice Intel/Microsft or any thing of this sort, I am criticising You and You idiotness for not checking the requirements for Windows 10] (If you are annoyed of the ammount of the thread with a "Pixel Format Not accelerated" problem, plz Share this signature
They aren't despawning, they aren't overcrowded and pushing each other out. It's just a glitch in the game. Fence corners are the worst. I've seen sheep walk through fences. I've seen them walking ON fences. Pigs too. I have the least amount of trouble with chickens, when they used to be the worst. I think it's because I now keep them inside the barn.
Which points to a solution: Walls. Either keep them in pits or keep them in barns. And make sure they aren't overcrowded.
Mob's can glitch outside of pens during loading. They don't need to be especially crowded or anything for it to occur. They don't push each other out, they just take a nice little stroll. Sometimes they can also turn invisible but that seems to happen more after I have to restart from dying(fixed by reloading the map).
The best luck I had was with a design similar to what rodabon was talking about(my own were floating). Leashing has also worked out for me(so far).
I've been wondering if a 2-thick wall might work as well. I've heard of people having good success with pits. That might be what is going on there.
I wouldn't have thought they could cross a 2 deep trench.
Try building a two block high wall instead of the fence (or inside it, not outside)?
Just testing.

I tried it with terrible results. I gave my wife my glasses for a second, a creeper showed up and now my wife is pregnant.
Stupid 3D..

I been using glass blocks since then and have had hardly any issues since, mind I am playing 1.7.4 (PC).
Mintutor now works in 1.13!
