My most recent death I fell while trying to get into a temple treasure room, and I hit the pressure switch and blew myself up about 500-600 blocks from my bed.
On my way back to get my horse with diamond armor I died.
Needless to say I started a new world that is going fairly well. XD
I don't play normal worlds very often, so I don't die normally often. But when I do, it's usually due to the evils of gravity or the dreaded lava. Damn nature, you're scary.

I Mostly play single player vanilla survival and rarely die anymore. I always carry water when mining in case of lava, and fire resistance in the nether, learned that lesson! Sometimes, early in the game the hostiles get just too pushy and things get a bit chaotic, or i fall into a ravine while running from them lol. But like i said, that´s just early in the game and a long time ago.
The last time i died was because of a very annoying wither, i died a couple of times and lost a lot of gear. He destroyed it. So, overall, common death is wither!
probably i die from fire more than anything although i die from zombies a little bit less commonly but more than creepers and skelis (i die from them because i underestimate them)

lol I don't die at all. If I were to die it would have to do with some REALY strong modded boss that arrived or was accidentally summoned early game(first day). Even then I would just cheese it by (hiding under a tree if its 3 blocks tall.) I currently have a hardcore modded world that i have survived on for 6 Months.
It's a toss up between falling or mobs gangbanging me. Falling not just as in a 1-hit death from super high up, but a series of stacking falls/other forms of damage, with a last fall taking me out for good. Big gangs of mobs too, such as a couple Zombies, a Skeleton or too, and a Creeper. Specially the last one, since I play with all sounds off
TNT cannons. Yes in survival single player. It helps me mow down mountains. But every dang time, I forget to place couple pieces in some places, and my 20x20 TNT goes to hell with me. It sucks. So many resources lost.
No mountains were harmed in these situations.

Witch/Skeleton/creeper trio in caves (I must be the luckiest person to have all three together in an inescapable cave system.

Elytra crashing into things by far.
