Support, like ect ect. As weird as it is, I think minecraft needs more potions like this. Potions that screw around with game play, giving additional challenge and mechanics to adventure around. Eg dodging Poly-slime potions in the middle of a river. You fail=you drown. XD
Oh, and this can be a splash potion! Now THAT is a way to mess with people. You can turn them into squids to slow them down or pigs so they get killed.
Umm... Most splash potions do the same effect as the drink.

Somewhere above me, someone said that hostile mobs won't attack you if you are disguised as a hostile. But what if you attack them?
I like this idea, if you were a cow, you could sneak attack people. I like the peaceful mob idea because no one would really botheryou. But how long would it last? is there a time limit? And if they hit you do you turn back into human form? I like the splash potion effect so you can troll other people, this will wreak havac on multiplayer. Half support, I just want to know if you're able to turn back into human form
imagine your sitting in your house and you see some noob killing all your pigs that livee in the plains nearby (or any farm animal for that matter) so you quickly brew a potion corresponding to the mob he/she's killing with the longest lasting one (8 minutes?) make it into a splash potion, walk up behind him and turn him into a pig/cow/sheep/chicken! then, you get your buddy to come on to "kill some pigs/cows/sheeps/chickens" kill all of them, and steal his/her stuff. or just throw another on him every few minutes. it would take forever, but it would be a funny punishment for killing your animals. i don't know, sounds like a cool thing to add to minecraft anyways.

Isn't Invisibility more effective? They can't even see you but disguises can be seen through.
You can see invisible people somewhat, and disguise potions are not only for being undetectable. If you see a cow, there's a slight chance of it being a person, but if you see pieces of a cloaked player or particles, you know it's a player.
I imagine that you wouldn't be able to mine or suchlike while polymorphed, and would instead have to drink milk to disable the effect (or wait for the effect to wear off).

i like this, but i think it should have some drawbacks, like the creature of origins skin being loosely applied to the mob they turn into, or maybe it only effects the mob you become, but if you get too close, the mob will realize your a phony and attack you.
For example, if you used a zombie disguise potion, skeletons will still attack you and if you stay withing a 2 block radius of a zombie for more than 3 seconds, the zombie will attack you. and it wouldn't be easy to trick a player because you would still have you skin, but it would be over a zombie.
It's an interesting idea, but I prefer mob disguise. Mod disguise already exists, and requires no client mod. Also, even if the potions are expensive, I believe that this idea is overpowered. You could possibly ask the creators of mob disguise to implement some of your ideas.
