I think you've narrowed it down pretty far as is with all those criteria, may as well named the thread 'rarest natural block in overworld' as you've taken away everything else. I'd say the rarest block is the tripwire hook, as I've only seen two of those in a natural dungeon, and there are a few more repeaters than that.
Uh, End Portal Frame. The game spawns 'em in the Overworld but there are 36 or less on the entire map, and that's a big map. Do I win anything? I hope I win cupcakes!

Trying to get emeralds is a pain for me, Playing on a server for a few weeks none stop mining i found 9. Rarest block emeralds.
Emerald mining is amazing!
I do recommend mining them with a fortune III pickaxe to maximize the emerald yield. On a legitimate survival server I have nearly 2 stacks of emeralds after a few hours worth of playing
I'd say the block I see the least is cake It's been over a year since I last saw a cake placed on the floor.

Mycelium. I have yet to find a mushroom biome legit.
I've found 2 legit mushroom biomes (And i'm glad, those things are f'n ugly)
But still the silver fish spawners have to win..
And I wouldn't count the dragon egg, first off...its not really a block, second you have to kill the dragon to get it... it isn't really "naturally spawning"
if you were to count blocks themselves and not different types, then sticky pistons may be the rarest. If you were to count different types of blocks, then a silverfish spawner may be the rarest.
It's not silverfish spawners because those naturally generate in Extreme Hills biomes as of 12w38a.
I think you mean stone silverfish eggs.

They spawn in jungle temples.
Cauldrons would be up there I would say.
All together, though, the dragon egg is the rarest if you were to count all dimensions.

Not in a server I played on, I was always SOMEHOW finding mushroom biomes when on the sea.
It's not the portal itself because the portal requires user input to spawn.
It's not silverfish spawners because those naturally generate in Extreme Hills biomes as of 12w38a.

It is a silverfish spawner. One in each world...Edit: Someone said that lol. I say it is emerald.
