1. Like steam, there is a base name and you can change your DISPLAY name
2. You are banned by base name
3. Fee's for it. You get your first change free.

I support the idea, but the reasons you gave to support it were absolutely terrible. A person should only be able to change their username to one that isn't already taken, and your last username will then be up for grabs.

How are they terrible? You're basicaly changing the words of #6, which I did not see that I messed it up.
People could run around spamming servers, and then change their name.
The only exception would be if you put personal information in your username, but that usually doesn't happen, and it doesn't really matter. I, myself even put my real name as my Minecraft username.
They won't do that.
The game would use a UUID, and use that for bans.
It would be:
Change username [username]
username is already taken!
So people can't use a name someone else is using.
however then there would be the problem of identifying people.
so implementing changeable display names with a command or some other way of checking the original username of people might be a better idea.
Some players that first made accounts in alpha did not know at that time that their user name would be their character name when multiplayer was first introduced. They saw it as a mmorpg that you could make another name for online play.
Heck, some players in Beta (*ahem*) did not know their user name would be their in-game name. I would love to change my in-game name to something more suitable to Minecraft. As long as there are proper safeguards to allay abuse, like the OP and many have listed before me, this has my support.

I am making a lifestyle choice, one that came long after I set up my minecraft account. The current account name is not in form with what I aim to become and serves as a grim reminder of what I leave behind.
