Go into .minecraft, delete EVERYTHING in there (back up your saves first!) then reinstall minecraft. When it's done, close minecraft and patch. Should work fine. Now put your saves and tex packs in.

Yeah I get the same problem but I found that after you get a black screen just close minecraft then open MCpatcher again and without changing any settings just hit patch again and then minecraft works 100% of the time for me but idk why.
thanks i didnt want to have to delete everything so thanks im glad i saw this

I get the same problem as many people here. 1.4.5 gives me a black screen with MCPatcher 2.4.3_04.
The problem lies within the "Better Skies" mod. If you uncheck "Better Skies" the patcher will work. If you install "Better Skies" then you will get a black screen of death.
1. Patch Minecraft with MCPatcher
2. Run Minecraft -> Black Screen of Death
3. Exit Minecraft
4. Close MCPatcher
5. Restart MCPatcher
6. Repatch Minecraft
7. Run Minecraft -> should work now
Huh thats odd but that worked too. Bug somewhere in there but its a workaround
EDIT:
Actually you can just:
- Run patcher
- Close patcher
- Open patcher
- Run patcher

Don't know why, but I've had to do this exact same thing since MC 1.2 . Black screen without fail after patching with MC Patcher first time, then works perfectly after the second patch using exactly the same settings. I find sometimes it's also necessary to run MC in between the first and second install or I still get the black screen. It's only a minor inconvenience and I still swear by MC Patcher.
Lol! Reading back, which I should have done to start with, it seems this problem/solution is quite common...reassuring that it wasn't just my set up.
I ran the .jar and when I test minecreaft it never fully launches, just stays on the black screen. im not sure i did anything wrong. Im running vanilla minecraft 1.4.5
For an odd reason I cannot install the 'HD Textures' mod on it. I can install the 'HD Font' but not the textures.
I do already have some mods installed, like TMI & Millenaire. I've never had this issue before when using Mod Loader, but now that I have switched to Forge, I seem to be getting this.
Took me ages to get these mods installed, restarting again would not be helpful or may I have to?
Kahr, the next version 1.5 will probably be official support for HD texture pack. And your patcher is no longer required. What do you do?
the patcher is more than just an hd patch now... it is a tool for artists that wish to expand upon the vanilla boundaries of the game where texturing is concerned. (all the included mods) not to mention since the api isn't going to cover mods that alter class files, the patchers ability to install those and recognize conflicts make it a suitable program for that.
about vanilla hd support.... it's been on the official wiki for a while now as a 1.5 feature, but I say "snapshot or it isn't happening".
Where did you get that information? I don't remember anything from Minecon this weekend that mentioned HD texture support.
Dinnerbone tweeted today that 1.5 would include support for HD textures and animated textures, but not the mod API. Also, 1.5 is targeted for a January release, so it's a ways away still.
The still leaves BetterSkies, CTM, RandomMobs, etc. that will still be needed even after HD texture packs are supported in the game.

I don't expect them to include all the features that MCPatcher or even Optifine include for HD Texture immediately and I honestly think they'll patch it a few times before they get it correct.
Been surprised at the errors they've been making recently with fixes.

agreed... keep in mind that all those errors are the reason they are re-writing the game before the api gets built.

Well, if the HD texture patcher will not be needed, connected textures, better skies, random mobs still needed.

Yes it does. Make sure to install forge first.
I've had some bugs with water/lava and portal textures but just uncheck HD Textures and it'll be fine.

And you forgot to say that NEI take ages to update, unlike TMI
