Would a few people try a test version of the patcher and let me know your results? There are fixes for the items-with-whiskers bug and the missing custom items from ModLoader. I'm especially interested if you are having the problem below:
So I downloaded and ran the beta patcher after a clean minecraft install, and when I'm not getting the MC out of memory error(which is more often than not) I load my save and still have the white blocks and i read through the output of the patcher and no obvious errors occurred so idk whats happening

Probably a ram allocation issue. I use 64-bit Java and set Minecraft to use 2GB of ram (rather than the default 1GB) and have not had any issues with the x128 and x256 texture packs
2GB of video ram in the system as well
I would suspect that high texture sizes would need large amounts of video ram.
forgive me for my ignorance but how did you do that?
-Xms defines the smallest allocation of ram
-Xmx defines the maximum allocation of ram
-jar defines the path location to your Minecraft.exe file.
Usually defined in #m for reference sake here. If you are like me and leave Minecraft.exe on your desktop then the above will do fine. You must have 64-bit java to allocate 2GB to Minecraft.

I can't check on the white boxes but I can tell you that the "item-with-whiskers" bug seems to be gone now.
EDIT: Looks like it may be causing issues with the MPMenu extension now.

Just an update on it. If I pull the Multiplayer menu (that MPMenu affects) it works around the crash. I went back and started narrowing it down since it only reproduces if I go directly to load a single player world (no other menus accessed)
I found the faulty combination.
Mods
ModLoader
MPMenu
The install order is ModLoader, MPMenu, then apply patch using MCPatcher
This doesn't appear to be a new issue either. I can reproduce this on the non-beta version as well. So this issue is present on 2.1.0 and 2.1.0_01-beta1.
Update: Nevermind... This issue is nothing to do with MCPatcher. It's a conflict between MPMenu and ModLoader... Ugh... And MPMenu relies on ModLoader... double-ugh...
Hmm, now when adding a mod like More Flowers the textures show up as just colored squares on both the default and the HD pack. Whatever color the squares in the terrain.png file are.

The op posted another version on the previous page that may fix the issue for most people. I know it fixed it for me. If you want give it a shot and report back on how well it works for you.
So I can't get these out of memory errors / white box items to happen for me no matter what I do. I'm using all three versions of the LB pack. Does enabling or disabling the texture cache in the MCPatcher options make a difference for anyone? Here are my specs and settings. How do these compare to those who are having problems?

I noticed that when i patched the game with the texture cache it gave me errors of out of memory.
but when i patched it without checking the texture cache it worked fine, just in case it helps
The test version worked for me when ModLoader textures weren't loading. Here's an error thing from the release version:

No. 64-bit code will not run on a 32-bit operating system.
