I thought it was worth noting that you can install Optifine straight into the jar, which is how I installed it, particularly since I install my own mods the same way; you delete META-INF in the Minecraft jar after installing Optifine, as detailed here; the installer simply automates the process:
It only takes me seconds to do that after I've made a profile; of course, I've done that countless times when updating/testing mods. Also, another method I used before is detailed here; the Optifine jar is added as a library, which can get Optifine to work if Forge doesn't like it for some reason; I also used this for my own base-edit mods back when when I used Forge on 1.6.2 (I even edited the Forge-patched source to ensure mod compatibility as otherwise the stuff Forge adds wouldn't be present) as I can easily update them by simply updating the jar file with my classes (nothing more than a zip folder containing some classes, renamed to .jar) instead of having to reinstall Forge if I removed any classes.

Optifine doesn't show blocks that are not in your FOV. If that's a big block, and you're not looking at it, it won't show up.
Is the Optifine settings in game like on the Video Settings thing?

- many performance optimizations are possible, but the complex framework gets in the way
- the vanila chunk loading has some problems, lag spikes everywhere
- anisotropic filtering has disappeared (?)

Interestingly, on my machine I haven't noticed any lag spikes in 1.8 yet... except when recently I created several new worlds, they somehow crept up from behind to explode in my face like the Bastrd Creeper From Hell, and from then on stuck like glue - persisting even across worlds - until I quit and re-started minecraft.
- Don't expect it to run faster than vanilla (but it may run much smoother)
- Don't expect all features to work (some do, some don't)
- Do expect it to crash and in this case please post the crash report (error log) in a spoiler

- placing stained glass blocks (panes) may produce ghost blocks until the chunk is updated again
- when placing blocks, the block bottom may flash for a single frame

- CPU single/dual/quad-core
- GPU Intel/AMD/NVidia
- FPS comparison with vanilla when standing still and when moving
- Smoothness feel (fluid, sticky, micro-stutter, etc)

Yes, Optifine changes all the Video Settings to its own. Again, use the OP, down near the bottom it'll tell you what to do if you're getting low FPS.
