Invisible chests, blank signs and twitchy pistons are problems I'm seeing frequently in the new version. Always loved optifine but it's pretty buggy for me as of late
I have contacted the maker of this mod with no reply. My FPS has dropped from being 80-90 to being 35-45 after updating to 1.4.6 Ultra. Same exact settings.
I've not gotten a reply either; course this thread almost gets too many posts to follow by a single person alone, especially when they are busy trying to develop mods.
There are 27,000 replies to this topic. But don't worry, I am sure he's going to ignore everyone else and get right on your personal problem, for which you've provided no system specs, background or description of.

I appear to have lost an extreme amount of performance using the latest version of Optifine.
I regularly sit around 400-600 FPS uncapped, but with Optifine (Ultra) installed and using near default settings (multi-core selected; irrelevant to FPS issues though), I'm sitting at around 150FPS (uncapped). When loading into the world my FPS is below 60 and a bit sluggisih/skippy when initially loading.
The world also goes black when I change some settings, requiring me to relog. I can see mobs, items and some terrain in places when this happens. http://i.imgur.com/EiydQ.png
OptiFine has always seemed to have issues for me every time I've tried it. The most common (which still happens mind you) was the terrain randomly flickering repeatedly, usually when moving or when objects move in the world (opening doors, etc.). However, the problem right now is unique and quite frankly causing worse playability issues than the flickering issue.
I've re-tested while screwing around with shaders (as they dropped me below 60 and I was wondering how badly OptiFine was hurting my FPS again) and my FPS dropped from 470 to 390, which is a lot better, but still shouldn't be happening when all settings are the same compared to without the mod.

Same here. It has something to do with the chunkloading for me. I used to be able to use multi-core in 1.4.5, but now when I use it in 1.4.6 I get horrible 'stutters'. Then my framerate drops to around 30-40 when I usually get 60-80 fps back in 1.4.5. Smooth chunkloading still works fine, but I still have about 20 fps lower than using multi-core on 1.4.5.
I want to remind EVERYONE in this thread that LAG =/= FPS. I want to strangle everyone who keeps saying "I have HORRIBLE lag in my single player game!" No such thing...
Lag has to do with packets being sent to and being received from the server.
Ex. I fire a gun point blank at someone, 3 seconds later, they die. THAT is lag.
FPS is 'frames per second', or in easier terms to understand, how many frames show up on your screen in one second.
Lower FPS doesn't have anything to do with lag, it has to do with rendering of the software with the hardware of your computer.
In some cases lag can result in low fps, but probably 90%+ of the time it doesn't.
Other way around that most see it, low FPS is NOT the cause of lag. Everyone learn your terminology!
Optifine does not work with X-ray! When I press X, it's works fine, but then when I try to go back to normal, It just shows caves!!! When I removed Optifine, it worked Perfecto!
Soooo this is one of my favorite of mods cause it increases my fps from 1-19 to 55-60 on a laptop with no integrated GPU.
But that's not why i'm here. I have found a slight glitch/bug that causes fire works to no longer work. I only have optifine and I have also asked around and see if anyone else has the same problem. I have particles on at all times and on fancy to test it out. I also reverted back to normal non modded minecraft to see if it worked, it did just FYI and now i'm going to shut the heck up cause I'm nerves and when iI'm nerves I tend to go on and on and on....

Lag exists in single player. First, MC operates on a server since 1.2 or so, even in single player.
Second, there are lot of lag problems that are not FPS problems. For example, riding a minecart can cause the world to freeze for a second then jump ahead, just like in your "shoot guy, 3 seconds later he dies" scenario. Sometimes pressing a door to open results in a lag spike that opens the door a second or two later than normal. Sometimes it appears that you have been stuck in one spot, only to find that you were really moving all that time, and it was just a visible lag -- not good when you hear yourself blowing up from a Creeper, but the lagged video stlll has you standing in a safe spot.
Lag and FPS may be different, but both problems exist in single player, and normally Optifine will help with BOTH.

Other objects such as Redpower Windmills and Thaumcraft wisps are also showing up as missing textures:
If I attempt to reload textures with F3+T Minecraft instantly shuts down with no crash log.
