Get rid of mcpatcher. bye deleting your entire bin folder (yes it's necessary) and install optifine and it will run.
And my Optifine version is D3
Same here.
It fixed it for a short while because it made the world generation faster but now that i have added some more mod that make the world generation longer it happen again
If you could please help or tell me a fix to this it would be appreciated. Thanks

It's still definitely related to your computer, I'm not commenting on your computer's, er, "good-ness."
Optifine doesn't have MCPatcher's Better Glass yet.
Turn your render distance to normal.
Can you post a screenshot?
I've bolded the important parts of the crash report.
The top and bottom texture IS applied correctly, but it doesn't affect the sides. I also tried changing the texture to see if it simply wasn't connecting/set up right, but it had no impact. I also tried adding faces=sides but nothing doing either. I went ahead and made it a terrain.properties file instead, and it worked fine, but it makes me think that ctm doesn't properly recognize adjacent blocks.
EDIT: I should also mention I haven't tried using McPatcher, so I don't know if its only a issue with your version.
The one you currently have it updated to is for minecraft 1.4.5 too but a slightly older forge version. I think he updates it like once a week.
HOW you keep up with that beats me <3 but kudos to you for that. What would you suggest I do? Do the Forge earlier 1.4.5 version then? If so, I'd assume most modders keep to the initial Forge version and don't update as much as Mr. Forgeman does?
oh god, I just saw, he did these 3 updates in 3 back to back days. Really???????? Yeah, there is NO way you all can keep up with that kinda thing.
What's the best road to take then? This guy is spitting out new forges every day it seems like; so what do you modders do? First main update he does for the newest mc version?

Anyway, I have a slight problem with Optifine...I have no other mods installed, and I'm on normal, but I can't get monsters to spawn. It was working fine before I installed it, but as of now my Monster Farm is completely empty. Are there any settings added with Optifine that would cause this?
Edit: Okay, I seem to have figured out the problem. I had tried to reinstall Minecraft completely, and after I did so, and reinstalled Optifine, monsters would spawn in my trap again. Happy, I went back to set my settings back to how they were. But after I finished that, monsters stopped spawning. Confused, I messed around a bit more, and it seems that having my render distance on Normal stopped monsters from spawning in my trap. I have it set on Far now.
More details about my trap: It's up at 190y so that I don't have to worry about things on the ground, and the actual spawning pads are about 25 blocks above where my character stands, with three six block high chambers (the area inside is only 2 blocks high, I'm meaning each entire structure including the blocks building it is around six blocks high). I dunno if it's because of the height I'm at, but at most 1 or 2 monsters would spawn above me (checking with F3) when normally I get 25-35.
Weird glitch or something else? Normal spawning on the ground works fine on normal. It's just things spawning on ground that is above me on normal that seems to be glitched.
Try to deactivate the Multi-Core/Smooth chunk loading, then restart your minecraft.
The Multi-Core chunk loading needs the graphics driver to work correctly when used from multiple threads.
Some graphics drivers can not handle threads correctly and show broken textures.
