I don't see any rank values in any of the groups except for member. Each group has to have a rank setting in it for the ladder to work. With PEX, the higher the number, the lower the group is.
Right now, you have 100 set as the value for the member group. A group that is numbered, say 90, would be the group that a player would be assigned to with a /pex promote <player>. A group that is at 110 would be the one if you demoted the player.

It's the PEX wiki, freshly redone. The command you need to set ranks for your groups is right there at that link. Using the command will also prevent any chance of a typo in the file formatting.
