What is the brand, model number, etc of your computer? Or if it's a custom build, what are the rest of your specs?This article on PCWorld might help you, but since you are asking the very general question of how to upgrade, then for you it's going to be very hard to upgrade it without knowing all the technical stuff. Also, if you are running Minecraft off a computer with HD 4000, then the rest of your specs must be pretty low, and if you are going to attempt Shaders, you probably should just really get a better Gaming PC. You will be able to play modded minecraft without upgrading, but with Shaders, no.

You tried to install Shaders on an Intel card. Pretty sure it doesn't work at all. Remove the shader stuff.
You other issue is you watched Youtube videos. They are always going to be uninformative, out of date, or simply wrong. Just don't go to youtube for information regarding anything. For each mod, seach this forum for the topic it was created in, then follow the installation instructions one by one.
Go to files.minecraftforge.net for latest Forge download.
Also, if your computer is not an up to date gaming rig, I wouldn't try to run more than a dozen or so mods. As far as the shaders part goes, I've never tried to use them, but I would just leave them out. You're going to need a good computer to run shaders.
Right now I'm playing on an HP Envy dv7, and while I'm not able to play really truly big modpacks, Yogcast might be one example, or with Shaders, I can play plenty of modpack just fine with Optifine, such as Project Ozone 2 or FTB Infinity Evolved, which are both fairly decent sized packs. So you can play modded even with a less than great computer, just maybe not one that's a decade old and probably not massive packs or with Shaders.
Try to make environment path variable for your java jre and update openGL manually. Or try to disable openGl right before starting a game. If you have more then one graphic card - try to use not integrated one.

How do I update my old graphics chip?

What is the brand, model number, etc of your computer? Or if it's a custom build, what are the rest of your specs?This article on PCWorld might help you, but since you are asking the very general question of how to upgrade, then for you it's going to be very hard to upgrade it without knowing all the technical stuff. Also, if you are running Minecraft off a computer with HD 4000, then the rest of your specs must be pretty low, and if you are going to attempt Shaders, you probably should just really get a better Gaming PC. You will be able to play modded minecraft without upgrading, but with Shaders, no.

You tried to install Shaders on an Intel card. Pretty sure it doesn't work at all. Remove the shader stuff.
You other issue is you watched Youtube videos. They are always going to be uninformative, out of date, or simply wrong. Just don't go to youtube for information regarding anything. For each mod, seach this forum for the topic it was created in, then follow the installation instructions one by one.
Go to files.minecraftforge.net for latest Forge download.
Right now I'm playing on an HP Envy dv7, and while I'm not able to play really truly big modpacks, Yogcast might be one example, or with Shaders, I can play plenty of modpack just fine with Optifine, such as Project Ozone 2 or FTB Infinity Evolved, which are both fairly decent sized packs. So you can play modded even with a less than great computer, just maybe not one that's a decade old and probably not massive packs or with Shaders.
OK so I found out that it's just animated player and shaders mod making my minecraft crash.... why?
Because your hardware (specifically your GPU) is to old. Do you have a newer PC you can use?

Sorry no I only have this one and a older one, it's ok though i'll try to sell this one to get a new one....
Try to make environment path variable for your java jre and update openGL manually. Or try to disable openGl right before starting a game. If you have more then one graphic card - try to use not integrated one.
