This assumes you know how to find/open said file.

1 --- If you are installing mods or using xau's HD Texture Fix, delete the METE-INF folder, (the whole thing), otherwise leave it alone.
2 --- If you are only adding mods skip to 5, otherwise continue to 3.
3 --- Drag any desired retextures into the opened minecraft.jar file, or into the appropriate folder within the minecraft.jar file.
4 --- If this is all you wanted to do, close the minecraft.jar file, and play the game, otherwise continue to 5.
5 --- If you want to add any mods, create a temporary file, call it whatever you want. Mod by mod, copy the files for each mod into the empty folder, this way you'll know if there are any incompatibilities. Once all of the files for all of the mods you want are in the new folder, select them all and drag them into the minecraft.jar file.
6 --- If this is all you wanted to do, close the minecraft.jar file, and play the game, otherwise continue to 7.
7 --- If you don't want to use the HD Texture Fix patcher, skip to 8, otherwise continue with 7. Place the HD Texture Fix Patcher in the same folder as the Bin folder, Resuorces folder, and Saves folder. (the same Bin folder that the minecraft.jar file is in) Double-click on the pacther. Click on the browse button next to the text box for "Output". Set the file name as "minecraft.temp.jar". Click Save.
8 --- Make sure "animated water", "animated lava", and "animated fire" are checked. Make sure "custom water" and "custom lava" are unchecked. Check or uncheck Better Grass as you see fit. Click "patch", wait for the program to run, and close the patcher.
9 --- If this is all you wanted to do, go play minecraft, otherwise continue to 9.
10 --- Place the Simple Map patcher and the mods folder that comes with it in the same folder as the Bin folder, Resources folder, and Saves folder. (the same Bin folder that the minecraft.jar file is in) Double-click on the patcher. If you did not use the HD Texture Fix patcher, skip to 11, otherwise continue with 10. Click on the browse button next to the text box for "Original" Select the minecraft.temp file you made in 7 and click open. Now click on the browse button next to the text box for "Output". Select the minecraft file and click save. If something pops up asking you to overwrite a file, agree to it.
11 --- Make sure you check "simple map". Check or uncheck "smchat" and "ambient occlusion" as you see fit. Click "patch", wait for the program to run, and close the patcher.
12 --- If this is all you wanted to do, go play minecraft, otherwise continue to 13.
13 --- Run the PatchMinecraft.exe that comes with the AutoMap download. Wait as the program runs, clicking yes/okay/whatever/etc when things pop up.
14 --- Start minecraft. Open a world. Once the world is loaded and you can move around within it, double click on the MinecraftAM.exe that comes with the AutoMap download.
15 --- Close the Auto Map before exiting a world.
Warning, last time I checked, once you use the automap patcher, you can't use the simple map and/or HD texture fix patcher(s), nor can you add any new mod files, unless you revert to a backup.

Rename the one you want to use to "overlay", you'll have to rename the one already has that name. Now go back to the simple map patcher. Double click it.
8 --- If you want to use the HD Texture fix also, continue with 8, otherwise go to 9. Now click on the browse button next the the text box for "Output". Name the output file "minecraft.temp.jar". Click save.
9 --- If you want to have better grass too, check that box, otherwise leave it unchecked. Now check the box marked Simple Map. Now Click Patch. Once it's done, close the patcher.
10 --- If this is all you wanted to do, play minecraft, otherwise continue.
11 --- Make sure you place the HD Texture Fix patcher in the same folder as the bin folder that the minecraft.jar file is in. Double click on the HD Texture Fix Patcher. If you used the Simple Map patcher before this one, continue with 11, otherwise go to 12. Click on the browse button next to the text box "Original". Select the file that you made earlier with the Simple Map patcher called minecraft.temp.jar. Now Click Open.
12 --- Now click on the browse button next to the text box "Output". Select the minecraft.jar file. Click save. If you want better grass, check that box, otherwise leave it unchecked. Now check and uncheck all of the other boxes as you see fit. Now click patch. Wait for it to finish, then close the patcher.
13 --- If this is all you wanted to do, go play minecraft. otherwise continue.
14 --- (I am testing the AutoMap right now, will add this part in later.

Hi, I'm on a mac (not a horse, lol) and I tried to install the mod Hoverboat, I located the .jar file and opened it with archive utility giving me a folder with its contents, I then added the new mod file in. The files in the META folder were also deleted. When I was done I had the folder and the .jar that I had used archive utility on but I know I must have messed up somewhere because when I load minecraft the mod doesn't work when it should.
Instead of extracting the files within the minecraft.jar file, right-click on the minecraft.jar file, and look for an option to open archive. This will allow you to add files into the minecraft.jar file without decompiling it.
