Because now, we modders not only have to mod the client, but the server as well!
This sounds easy, but Risugami never made ModLoader for servers, and it's much more difficult to pass information along. Also, it's twice the work!
So please, don't annoy us mod creators to update more quickly; we've got a difficult task at hand.
(If people nag at you and you're a modder, you might wanna link to this at the top of your thread)
You don't have to do anything server side. ModLoader takes care of interacting with the local server. Just code as you usually do. If you're making a mod that actually works for SMP, then yes, you'll need to code the server side as well, but not if it's just for SSP.

Nope.
Examples:
• You get an internal server error when you use a custom gui.
• Items that the server doesn't know are useless and get replaced as soon as the server sends the client new data about the slot they're in.
If you're just coding for use with the local server, you shouldn't be having issues with any of that. I directly took code that I used in 1.2.5, and it works just fine. GUIs, items, everything.

You are using ModLoader, right?
