this means that the data on disk is different then what the mod expected, but tbh i never realy did anything in that part of the mod, so i dont have a idea how it exactly works.
as for the missing grass colours i tried it and it seems that nog all biomes from the LOTR mod are found when the colours get loaded. so when the map is drawn the normal texture is used without a overlay.
i dont have acces to the mod code so i cant tell you exactly why it goes wrong, but seeing from the spam i get from forge about the mod doing things horibly wrong with registering stuff my guess is that it has something to do with that.
as for a solution i would advice to add your own overwrites for the missing biome numbers.
i personally always disable the saving region data to disk in the mapwriter options. (that part of the mod has some odd bugs, and i never regen the map so it's useless for me)
i can safely delete the stuff in there.
if you delete the images folder the map will be reset to default.
if you delete the mapwriter.cfg you lose all markers for the world.
if you delete the region folder you will be unable to regenerate the map from disk (pressing R in full screen mapmode)
Experiencing crashes while using MapWriter 1.9.4 - 2.5.19 in combination w/ Optifine 1.9.4 HD U B4.Optifine and Mapwriter work fine separated from eachother.

When using the MapWriter-1.9.4-2.5.19 with forge .1951 and Optifine 1.9.4 HD U B5 it crashes on me. It works fine on its own though

ive have found a little bug, when you hover over items on certain servers to find what faction they are in or what enchant they have when they do [item] inchat for example it shows a black box. http://prntscr.com/bntast can you fix this please.
i dont know what this has to do with mapwriter, i dont see anything conserning the map on the picture and i dont do anything with the chat
You are not supposed to extract anything, just like the first error message tells you. To install a mod, you just place the jar file inside the mods/ folder. And to delete the old block colours files, you should find them inside the config/ folder inside your minecraft directory.
If you extracted the jar file into the mods/ folder, then you probably have a whole mess in there at the moment... And that is where the duplicate mods are coming from, you have the actual mod jar in there, and then also an extracted copy of it, which forge finds and considers a duplicate.
My whole map is weird. I think it is because I started with a texture pack. The whole map is pink and yellow. My texture pack isn't and I have no idea why it is like that
Read a few posts back, you may need to remove the block color and block color overrides files and then re-explore those areas of the world to get the mre-rendered.
Hi I'm having a problem where my map doesn't seem to "save" between games. It was working ok, but every time I load my game it resets to my first session. Any land I explore is gone, any new waypoints I add are missing. It's like something isn't saving between sessions.

This is a recent development for me as well, doesn't matter if I create new worlds or try old ones, each time I load I'm getting the current area showing where I am, not where I've been.
you are right that it didnt apply the grass shading, the grey is the texture colour of grass on wich a shading is applied depending on the biome.
my guess is that it doesnt recoginize the grass block in those biomes as grass, and thus uses the default texture without shading.

for the people that are having crashes when installed with optifine, disable fast rendering
delete the block colors file, that should fix the grass.
as for the glitching that is an ongoing bug, if you log out of the world and back in it should be back to normal

do you exit minecraft properly?
check logs if there is a error with the saving of the maps.
Always just save and quit to the title then quit minecraft, no weird errors in the log that I can see.
