Alternatively, you could go up or down - Keyholes in the ceiling or the floor which only respond to the higher tier keys. Having layered dungeons like this would have the benefit of making maps more difficult to use, but might cause even more zombie noise spam, which is another issue I forgot to mention the other day.
So, honestly, it'd be up to you what option would work best. This is just my few cents.

I think he encountered a concussion creeper from EnderZoo (http://minecraft.curseforge.com/mc-mods/225247-ender-zoo). They teleport entities around them when they explode. I've been teleported above the bedrock ceiling by them, there are blocks of diamond scattered around, and no way to get back to the portal (without another mod like JourneyMap, a Mystcraft linking book, the bedrock-destroying TNT trick, creative mode, etc). If you have keepInventory set to true, or just don't mind losing your stuff, you can also make a tower of diamond blocks and jump off of it to die and respawn at the portal.
I am also finding this on my server since we updated to FTB Infinity 1.1.0
Is there a way to correctly reset the dungeon system? Or do i just delete the dimension and let it regen when we enter again?
Simply delete DIM-34, there is no proper way as of yet.
Is there any secret rooms in this dungeon mod as of now? And what about a lock in the middle of a room (revealing a staricase down) signifying that there are more rooms below? How about a room with a big chest with more treasure, but a dead end at the same time?
My apologies for being stupid here, but...Where can I see the changelog for previous versions before the most recent update? I upgraded through a few versions, but all I can see was the key recipe got fixed.

Yeah, I don't see any real changelog. It'd be nice to know if the issue where breaking the portal with a sword causing the server to crash is fixed.

Not at all. Offscreen - One of those Equivalent Exchange Energy Condensers is auto-crafting cakes.
