Reading NeonIsNeon's post again, it seems like VoxelMap is waiting for a ChunkUpdate, which is something that happens when a player enters the chunk, before re-drawing that chunk on the map.
If you close Minecraft and re-open it again, are the the black boxes back?
I suspect one possible cause for the server telling your client that chunks are empty (and causing the blank squares as a result) is that these servers have a set limit on the chunk rendering distance, and any chunks that your client tries to render beyond that limit -- if your render distance is higher than the limit -- generate the squares. I don't have time to test it out myself (had to refresh my pc today to flush out some nasty malware that also unfortunately killed most applications on there) but it's definitely something that you could try, and I'll experiment myself once I can get my GPU to play nice again.

To the Crashin issue ppl above talking about:
Minecraft keeps crashing after the newest nvidia g-force driver update, when voxelmap is installed. Many other users on my server reporting the same issues. Could you figure out what the problem is? (Minecraft 1.11.2, Newest forge and liteloader is installed also Optifine).
Something different i may ask, would it be a problem to roll out a "Fair Play" version of this map, without the entity radar and cavemap?
It's something that happens on certain servers. https://atherys.com/ is another one. I've never been able to pin down why. The client is being told those chunks are empty. I'd just ignore those but on some maps chunks really are empty. I don't really feel like developing heuristics for determining when to trust the info coming from the server.
If server owners disavow any knowledge of it, some plugin is doing something weird. I'd be curious to know which one
I'll check out the server linked, but if it's like Atherys, I'm not going to spend time in survival to try to diagnose things. MC itself is a bit of a chore, I don't feel like actually playing it as part of figuring something out haha
And yeah you can't get close to them without them getting painted over with the live data the client sees. When the client gets farther away though, server's saying "this chunk is empty now lol". They change color because that's the color of the sky. You go above 64 it's blue, below it's black. Same would happen with any empty chunk. I'll stress this again: it's not that the chunk is unloading, it's that the client is specifically being told it is empty
It would be really appreciated if you developed heuistics for determining when to trust the info coming from the server (I don't even know what they are haha). I can help to diagnose things if that would be able to fix my issue. I just really want to fix this issue because it's been driving me crazy

Ye it dosen't stop preventing the black squares from popping out no matter what render distance I choose.
Could you do the same for two more of those chunks?
Looking at your map, the black chunks seem to occur every 8 chunks. Your chunk is at x -16 ( = 8 * -2) and z 152 ( = 8 * 19), so that seems to fit my hypothesis. Of course this black chunk seems to deviate from that pattern.
Maybe you could report this to your server host, because, like MamiyaOtaru says, the problem is very likely to originate on their side.
if he isn't motivated to fix it himself, a list of plugins he is using would be nice. He can skip private ones only he uses because Atherys probably wouldn't be using them heh. It's something both of them have.
Someone would then have to setup a server and bisect search the plugins (remove half at a time until it goes away, etc).

I would encourage you to try it because it takes most of the mystery and "magic" out of setting up and running a server and gives you an /op eye's view of the things we see. You can then see what people like @MamiyaOtaru needs to more easily help with questions of plugins and mods. Once you've done it, it's no big deal, and it's really pretty cool!
On many servers, you may use /plugins to get a list of plugins. Some have that command blocked. http://mcstats.org/ appears to be down, but I think you may be able to use sites like that to actually look up plugins on other servers unless they specifically took the time to block it, and many don't even if they blocked /plugins on the server.
"bisecting plugins" simply means turning about half of them off to see if the half still left *on* causes the problem. If it works with half turned on, it must be in the group you turned off. Keep a running list and turn on about half of what's left. If the problem returns, it must be one of the ones you just turned on. It is a great deal of annoying work if you have a huge list of, say, 50 plugins. Eventually, you will find the plugin causing the problem.
If the server owner isn't willing to work with you because you shouldn't be using VoxelMap in the first place, you have to decide if VoxelMap is worth more than being dishonest with the server op that doesn't want you to use it. And even if you did find the specific plugin that is causing the issue, what do you do? "Hey ServerOp, could you fix this one plugin causing my VoxelMap to not work correctly? You know, that mod you don't want me using?" Eh... yeah.
Or if the ServerOp doesn't mind if you use VoxelMap but won't work with you to figure it out, you have to weigh that against finding another server where this isn't an issue. Or now that you know how to compile and run Spigot, maybe starting your own. >:)

I have just spoke to one of the admins and he said that all of their plugins are custom coded so its pretty weird how atherys and wynncraft has the same issue. He also doesn't want to fix it because it's not their mod so they don't want to spend the time fixing it.
@NeonlsNean: Correct, so you get to decide what it's worth and vote with your feet. I did, and all I got was a monthly bill for the privilege of running my own Minecraft server as a part-time job. At least I don't have to deal with admins not wanting to fix some quirk in their server or petty attitude from staff. I am the staff. :-p

fix for guardian elders (1.11+)
fix for players or mobs wearing plain Steve skull (crash fix for 1.8.9 and below, just visual otherwise)
screens with filters (waypoints, mob select and player select) start with those filters focused. IE you can open the waypoint list and start typing
support for a lower res background map that shows the whole server (not just areas players have explored) if it is included in a server resource pack (or elsewhere)
new keybind: toggle in-game waypoints on/off (unbound by default)

Not entirely true -- there's a server plugin that allows you to restrict certain features of voxelmap, as well as other LiteMods that support this feature.

So the above information is all i have for this "bug". Voxel Map just started crashing.
Java is up to date

fix for guardian elders (1.11+)
fix for players or mobs wearing plain Steve skull (crash fix for 1.8.9 and below, just visual otherwise)
screens with filters (waypoints, mob select and player select) start with those filters focused. IE you can open the waypoint list and start typing
support for a lower res background map that shows the whole server (not just areas players have explored) if it is included in a server resource pack (or elsewhere)
new keybind: toggle in-game waypoints on/off (unbound by default)
Not entirely true -- there's a server plugin that allows you to restrict certain features of voxelmap, as well as other LiteMods that support this feature.
