Start with a clean minecraft.jar and install OptiFine only.
Odd. All I have is Better Than Wolves and its requirement Modloader. I'll try again.
Edit: Must be some in-compatibility problem with BTW and Optifine, works fine with just Modloader and Optifine, But when BTW comes in it messes with some of the textures on tools and other things.
I've tried every version of Optifine and none of them work on my computer. I login and then I get a black screen (when the Mojang logo should appear). I know I've properly installed it (used 7-zip, deleted META-INF folder, transferred all files over to minecraft.jar). I even used a clean minecraft.jar (no other mods).

NVIDIA GeForce GT 640M LE 1 GB hybrid graphics card

Are you runing the game with an EXE or JAR file? If JAR, then you need to create a batch file to allocate memory to Minecraft.

That would be the launcher. It's usually not the cause of problems like this, so it's probably the minecraft.jar itself.

Then it should be a problem with the computer hardware. Unfortunately I'm not an expert at that so you'll have to wait for someone else to reply.
BTW I hope that someone can fix the "blocks lighting up bug". I never use AA this time and it still happens.
I have some instability here on hd_u_b2, not sure if this is even an optifine problem, since vanilla show similar behavior. whenever the game loses focus, like alt-tab to some other app, memory usage just starts to balloon out of control (regardless of heap size). sometimes it starts a neverending cycle of hdd swap + stutter, or may even crash. this is the first time I saw such an error though, with only optifine running on 1.3.2, no other graphic mods besides dynamic lights:

OK, now I've discovered that even though I didn't enable AA (is it Anti-aliasing?) I still have the "blocks lighting up" problem. Is there any solution for this? Personally, I don't think it has to do with brightness...
Update (or reinstall if already updated) your Java. If you're on a 64-bit Operating System, install/update both the 32-bit and 64-bit editions of Java.
Update your video card drivers.
Reboot.
Use CCleaner to safely clean out old registry entries etc.

EntityRendererProxy is not part of OptiFine, the problem is somewhere else.
It says to disable OpenGL Threading/Threading Optimzation...I'm not seeing this anywhere on the Catalyst Control Center settings, and I'm tired of trying to Google a solution as I keep getting too many nVidia references on the same page. So, anyone know how this is possible withing CCC? I'm hoping for a solution because changing it to multi-core actually makes it run worse.

quick query regarding the handling of custom texture packs... I recently (ie, about an hour ago) tried adding the battered old stuff pack, which in and of itself works fine - but whenever I enable Connecting Textures, sand turns into a nightmare gridwork of moire patterns and pixel-wide views of the underworld.
Mipmapping alleviates the problem slightly, but nowhere near enough. Is there any way of fixing something like this without relying on an update from the texture pack developers?

EntityRendererProxy is not part of OptiFine, the problem is somewhere else.

You patched your minecraft.jar with MCPatcher?
Start with a clean minecraft.jar.
If you also use Forge, it will say 1 error, but it will work fine.
