So I downloaded A5, I read the OP, did all the steps there (java 7, updated the lwjgl) and my fps went to 20-40 with Sphax128x. Now I've got a pretty good computer and was easily getting 70+ fps before, running all the same mods and everything.
To test that I'm not crazy, I went back to MT_A3 ....
... Important point: you're using HD_A5 , *singlethreaded* as a multithreaded version isn't out yet, but you're using MT_A3, *multithreaded*.
So for a fair comparison you should be using the plain single threaded A3, not the multithreaded version.
Because if you're comparing single-threaded to multi-threaded on a multi-core computer... well, its going to be slower. Thats just the way it works =p.
I am back to using Standard, after trying to tweak Multicore in every possible permutation to get the freezing up and transparent blocks to cease. No luck.
Following the OP instructions (turning off threaded optimization, turning on VSync) doesn't work. Forcing these options in Nvidia control panel buys you some time, as it seems to delay the eventual invisi-blocking, but it eventually returns and nothing short of a reboot (refreshing the drivers?) seems to fix it.
Black blocks do NOT go away when brightness is adjusted, either. Tried all brightness settings, nothing worked.
My setup: running only stock textures, no mods at all except for OptiFine. Only playing SP in survivor mode. Core i7, win 64 bit, with Nvidia GeForce GT 540M on a Samsung RF511 laptop. Most settings are set to fast, so a lot of details are turned off; rendering distance is set at a paltry 16. It's not ideal, but it works without overfrying the system and causing a hot-shut-down, although under Standard I am back to struggling with about a minute or two of lag during the autosave (which I have set to 30 minutes.)
In short, Optifine is the only thing that makes Minecraft actually USABLE, so it's a boon. But still not 100% there yet. That says more about Minecraft's code, than any criticism of OptiFine. it really is irresponsible that Minecraft has been around this long, is now a fully out-of-beta paid product, and still has the memory usage problems of an Alpha release. I'll take improved memory management over new "cat behavior" please!

I've seen that in mods that auto 'check for updates', and since I'm usually in fullscreen by the time they do their checking, I miss my firewall dialog asking me to allow the connection, so the connection is never made. Most mods that 'phone home' should have a way to disable that behavior.
As for the MT_3 / A5 comparison, several (if not all) of the mods you listed were updated after MT_A3 came out, which means, you're either using new mods that break old (A3) Opti, or old mods that break new (A5) Opti. By break I mean 'not optimized for'.

I'm experiencing a Java crash since updating to Optifine Multicore A6. I'm running Forge 3.1.2.94. I don't know if it could be related, but since Optifine is the only thing I've changed in my setup...
It seems to happen randomly as far as I can tell. =/ Am not sure what else to look for.

Also, no matter how many times I reload a 32x texture pack, sometimes the tops of chests are missing, sometimes it's the bottoms. The missing bits also appear in inventory. Sometimes, several limbs on mobs are also missing.

MT_A6 is still showing sings of a problem with HD textures. For me, it's usually mobs that are missing body parts, or they display all the textures at once. Thing is, it never happens the same way twice (uninstall, play, reinstall, different results). Sometimes everything looks fine until I hit the mob, then a random body part disappears, reloading the game (or textures) doesn't solve it. Oh well.

@sp614x, using the new A6 release there is still handheld item texture corruption when using 32x or higher textures when running better than wolves. I know you mentioned you were going to see what changed from the original preview of 1.2.5 A and later. Just letting you know just incase you might have started working on it. As usual thank you for the update, good stuff.

I have a fairly good computer and I used to get pretty good FPS on minecraft (going downhill since ~1.6), but when I installed optifine they got worse, not better, and i've had problems where certain parts of the ground will turn transparent temporarily, allowing me to see through to the ground. what's up? (using optifine A2 multicore on a MacBook Pro 13" with 2.66 GHz core 2 duo and NVIDIA GeForce 320M). If anyone could explain this, or even better explain a solution, it would be much appreciated. thanks so much.
First of all, you're bound to have problems running games on a mac.
That said, the disappearing chunks should be fixed by doing this:
Go in the Nvidia control panel, go to "Manage 3d settings", then go to threaded optimization and turn it off.

No, because games stopped being made with opengl a LONG time ago. (Other than minecraft) and even if it was, they'll run just fine
